Cardiology Pathway Coordinator

The post holder will be required to work independently, but under the guidance of Cardiology Consultant body and the Cardiology Service Line management team to facilitate patient referral pathways across Cardiology Service Line in sub-specialties of Heart Failure, Congenital and Electrophysiology.
The post holder will be required to monitor referral and triage pathways for the sub-specialties of Heart Failure, Congenital and Electrophysiology. They will also be required to monitor diagnostic test actioning, flagging amber and red alerts within UHP Trust protocol timeframes and monitor diagnostic test review turn-around across the service line.
The post holder will also be required to act as conduit for referrals between Cardiology and Cardiac/Thoracic Surgery to ensure completion of referral process and commencement of next treatment stage in timely manner.
***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

To navigate and support patients through various Cardiology pathways using own judgement and initiative, utilising specific departmental and patient administration systems whilst ensuring strict adherence and compliance to all Trust policies and procedures.
To maintain accurate referral and triage pathways for the Cardiology service line, working closely with relevant Consultant teams to achieve this.
To co-ordinate appropriate review of investigations, signpost patients to appropriate support and assist in ensuring the delivery of a high-quality service for patients.
To utilise iPM/CRIS and other systems/databases to track patient pathways and treatment outcomes and update accordingly.
To analyse data and to prepare reports for the Consultant and management teams to help develop the service line further in line with the Outpatient Transformation process UHPNT is adhering to.
To ensure monitoring systems align with the service line’s strategy and keep in line with set governance arrangements specific
to referral wait times and diagnostic reporting targets.

- Work with a range of clinical and non-clinical staff within the Cardiology Service Line.
- Provide and receive complex information about patient care and interpret instruction to relay to varying recipients specific to clinical referrals or clinical results.
- The post holder is expected to liaise with the Cardiology Consultant body to ensure appropriate navigation through specific referral pathways.
- To have a good knowledge and understanding of DRSS and UHPNT referral processes, and referral targets matching outlined NICE guidelines (specific to Heart Failure).
- To understand basic medical terminology relating to Arrhythmia, Heart Failure and Congenital to enable timely and accurate relaying of information,
- To work closely with other team members when chasing outstanding actions and processes, ensuring strong and clear communication throughout.
- Following up and tracking outcomes of requests and acting on these in a timely, appropriate manner.
- Provide basic advice and refer on or sign-post to other sources of support.
- Co-ordinate and organise reviews as required and to exchange that information clearly and concisely dealing with anxiety, cultural or language difficulties.
- Plan and organise complex programmes for co-ordination of patient pathway.
- Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
- To ensure that high standards of documentation and care planning for patients are maintained.
- The post holder is expected to liaise with the Cardiology Consultant body to ensure appropriate timely review of diagnostic test reports.
- Provide and receive complex information about patient care and interpret instruction to relay to varying recipients specific to clinical referrals or clinical results.
- To have a good knowledge and understanding of Imaging booking and reporting software and processes.
- To work closely with other team members when chasing outstanding actions and processes, ensuring strong and clear communication throughout.
- Ensure accurate data is rapidly captured and recorded to meet the standards of the management of patients, and to fulfil the relevant information requirements.
- Create databases to collect and monitor information, maintain and update.
- Facilitate collection and analysis of data by collating information via various electronic systems and face to face contact with members of the relevant Clinical or Service Line management bodies.
- To be proficient in the use of CRIS, CVIS and iPM systems.
- Works closely to develop good working relationships with Imaging Team, Fast Track Co-ordinators, Secretaries, nurses, consultants and the wider Cardiology Service Line.
- Develop appropriate systems to manage their own workload.
- Operates on their own initiative whilst bearing the overall directorate aims and responsibilities.
- To ensure that a clear understanding is obtained surrounding data quality, data protection, patient information management systems (iPM) and be able to work within these guidelines at all times.
- Ordering patient information and updating as necessary.
- Assist with teaching and educating other members of staff.
- Ensuring that patient experience remains the focal point for service improvement.
- Achieving UHPNT specific targets relating to referral triage, Referral to Treatment, DM01 diagnostic targets and governance processes/protocols.
