Job overview

 An exciting opportunity has arisen to join the busy Clinical Coding Department as a Trained Clinical Coder.

At UHP the Coding Department applies clinical coding to approximately 157,000 inpatient and day case episodes per year.

  • The post-holder will be a member of the Clinical Coding Department, which provides the Clinical Coding function for the Trust.
  • The main job purpose is to code the diagnoses and procedures related to the complete range of hospital specialties, for both inpatient and day case episodes, to agreed standards of accuracy, timeliness and completenes.

We have an Experienced Approved Clinical Coding Trainer and an Approved Clinical Coding Auditor in the team, meaning that full audit, validation and training support is available

The successful candidate will embrace the challenge of working in a highly demanding environment where excellent accuracy and attention to detail skills are paramount.

Applicants must be well organised and be able to work as a part of a team or on their own, with good time management and communication skills.

We also offer a limited Working from Home Rota and are working towards implementing an EPR system which will create the opportunity for further flexible working.

Main duties of the job

We require a highly motivated and enthusiastic person to join our team. The post holder will be expected to code inpatient and day case episodes of care, for diagnoses and procedures to high, internally-set standards of accuracy, timeliness and completeness. Applicants should possess a good knowledge of anatomy, physiology and medical terminology. The post holder must be able to work under pressure whilst maintaining the same degree of accuracy.

***Preference will be given to internal Trust staff, as well as Priority and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Detailed job description and main responsibilities

To demonstrate proficiency in extracting diagnoses, procedures and interventions from health records (paper and electronic), for all inpatient and day case episodes.

To classify all relevant diagnoses, procedures and interventions in line with national standards, local policies and local guidance, using ICD 10 and OPCS4 classifications.

To accurately input every code onto the hospital PAS system.

To code any outstanding uncoded episodes as identified by computer generated lists so that work targets set by the Clinical Coding Manager and wider trust are met.

Clinical Coders are expected to work towards the National Clinical Coding Qualification (ACC).

Clinical Coders are expected to partake in clinical coding audits and be measured against DSPT (Data Security and Protection Toolkit) requirements (90% accuracy for primary diagnosis and procedures and 80% accuracy for secondary diagnosis and procedures)

Band 4 Clinical Coders are expected to maintain or exceed a productivity level of 30 episodes per day.

Clinical Coders are expected to maintain accurate reporting of histology results.

All Clinical Coders are expected to ensure accuracy of data through data quality reporting and identifying incomplete information in the medical records and requesting information where necessary.

To liaise with clinicians, business managers, information analysts, finance team, clinical effectiveness and audit team and medical secretaries to address any queries arising during the abstraction process.

To assist the Clinical Coding Service Manager and the Head of Business Admin and Projects, in providing support through the accuracy, timeliness and quality of coded data to enable benchmarking.

 To undertake and attend intensive training covering all specialties in order to be able to change and adapt to new guidance as required.

To keep abreast of current national developments pertaining to clinical coding. 

To participate in clinical coding projects and meetings where necessary.

To support the Admin team on an ad hoc basis where needed, as directed by the Clinical Coding Service Manager.

Essential criteria
  • • Proven knowledge and demonstrable experience as a Clinical Coder
  • • Demonstrable experience in Clinical Coding using ICD10 and OPCS4

APTITUDE & ABILITIES

  • • Ability to abstract, analyse and interpret complex facts relating to patient’s clinical records

Qualifications

  • • Must have completed a Clinical Coding Standards Course.
  • • GCSE in English, maths and a science subject levels 9-4
  • • Or equivalent demonstrable experience
Desirable criteria
  • • If more than three years clinical coding demonstrable experience to have attended a Clinical Coding Refresher Course
