Stroke Advanced Practitioner

We are seeking highly motivated and experienced Stroke Advanced Clinical Practitioners (ACPs) to join our dynamic and growing regional stroke team.
In this role, you will play a crucial part in delivering high-quality stroke care, working closely with multidisciplinary teams to enhance patient outcomes and support our vision of a truly integrated stroke service.
The service is recruiting to 3 ACP roles, that will be a blend of 8a ACP and B7 Trainee ACP, and priority will be given to the right candidates across both.
***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

• Lead and coordinate the management of patients presenting with stroke symptoms, ensuring timely interventions.
• Following initial treatment, lead a thorough stroke assessment to identify and address risk factors, aiming to modify these risks and prevent future strokes.
• Be accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions, and responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations.
• Be an independent and supplementary prescriber. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of acute medicine, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.
• Contribute to the either tier 1 (SHO-F2, ST1-3) or tier 2 (middle grade/ST3) rotas, the work pattern of postholder being determined by their level of experience and competence. There will be opportunity as experience is gained to advance from tier 1 to tier 2.
• Collaborate with medical teams to implement advanced clinical practices and protocols.
• Participate in continuous quality improvement initiatives and service evaluation.
• Engage in research activities to contribute to the advancement of stroke care.

Person Specification
· Masters Degree in Advanced Practice or equivalent (as defined by Trust AP policy) alongside advanced practice training role.
· Statutory Professional Registration.
· Significant contribution to a Portfolio of Evidence
· Advanced Life Support/ (or Paediatric/Neonatal Life Support if applicable
· Independent non-medical prescribing

- Extensive experience within Stroke Medicine or clear identification of significant transferrable skills based on extensive experience in another field.
- Excellent clinical and practical skills
- Recent and relevant CPD relating to stroke care
