Job overview

The Colorectal Cancer Support Worker will be based within the Colorectal Nursing Team’s establishment.

This role will provide support to the team of specialist nurses and to the patients accessing this service.

The post is predominantly a patient facing role, most of the role will entail providing support to patients via email or telephone, and/or signposting to appropriate services for patients and their families/carers. It will also entail organising and assisting the clinical nursing teams to provide the Cancer Nursing service effectively and efficiently ensuring patients queries and care can be actioned within a timely manner. Processing queries and referrals, guiding and signposting clinical queries and patient queries.

This is a new post to Colorectal Cancer Services and aims to improve efficiencies within the pathways of care for patients diagnosed with Colorectal Cancers.

To provide general, specific and specialist information to patients and clinical teams regarding Colorectal cancer and cancer services, to ensure patients are always signposted during their treatment and beyond. 

Provide information and support for people with non-complex care needs.

To engage with the community services as appropriate

Recording patient information on the Trust’s Systems

Contribute to maintaining and organising the day to day running of the Colorectal service dealing with phone calls, referrals, emails, patient queries etc.

  • To provide general, specific and specialist information to patients and clinical 
    teams regarding LGI cancer and cancer services, to ensure patients are 
    signposted at all times during their treatment and beyond 
  • Provide information and support for people with non-complex care needs
  • Engage with the community services as appropriate
  • Recording patient information on the Trusts Systems 
  • Assisting with admin tasks related to the CEA Tracker to ensure the smooth 
    running of this service, ensuring blood tests are requested as required, 
    documenting on the appropriate IT systems
  •  Contribute to maintaining and organising the day to day running of the LGI
    Cancer Nursing service dealing with phone calls, referrals, emails, patient 
    queries etc.
  • Carry out holistic needs assessment (at times using e-HNA on an iPad) either 
    by telephone or face to face and signpost according to patients’ concerns

Qualifications and Training

  • Diploma or equivalent appropriate qualification, e.g. foundation degree assistant practitioner/nurse associate

  • Courses/further study attended to demonstrate evidence of personal development

Knowledge and Experience

  • Direct patient care. Experience of working in clinics and/or outpatient or inpatient setting
  • Knowledge and/or experience of working directly with people with cancer
  • Administrative and organisational experience
  • Ability to support and communicate virtually
  • Experience of dealing with routine and complex problems and escalation

  • Proficient in the use of hospital computerised patient systems
  • Evidence of using Microsoft Teams by way of supporting patients
  • Knowledge and/or experience of the Holistic Needs Assessment
  • Business and Admin experience in systems such as Microsoft office, Microsoft
