Assistant Practitioner - Liver Team

Job overview

The Assistant Practitioner will function at a higher competency level than that of other Healthcare Support Workers with responsibility for actively participating as part of the Multi Disciplinary Team within the ward/dept, to provide a seamless service to patients with proximal supervision from a registered nurse/practitioner.

The Assistant Practitioner will provide general/specific care as specified below for a specific client group and will be competent to work across professional disciplines, referring patients for Allied Health Professional assessment and treatment or to other agencies as required

You will work in support of qualified nurses, doctors and allied health professionals within an inpatient setting , outpatient and community setting.

Main duties of the job

·         To provide general and specialist information to a variety of patients with an alcohol use disorder and or diagnosed with liver disease.

 

·         To develop therapeutic relationships with patients.

 

·         To have knowledge of the impact alcohol has on health. To provide brief interventions, psychosocial interventions and motivational interviewing as appropriate.

 

·         To screen patients in the hospital to identify those at risk from alcohol harm.

 

·         To support other members of the Alcohol Care Team ACT in the care and management of patients.

 

·         To work closely with clinical nurse specialists undertaking a variety of skills Ie Phlebotomy, venesection, fibro scans, full training will be given.

 

·         Contribute to maintaining and organising the day to day running of the ACT and liver nursing service dealing with phone calls, referrals emails, patient queries etc.

 

·         This is a Monday to Friday service subject to change

This post involves working across the inpatient and outpatient service for the ACT and Liver team. Caring for patients in a variety of settings. Shift patterns are negogiable, but may include long days/nights/day unit hours/weekends/bank holidays.

The services takes referrals from across the hospital and also  runs  nurse led clinics Monday to Friday in the acute hospital and community settings.

After a period of supervision you will be reviewing patients on the wards and independently running a variety of clinics with set guidelines.

  • Foundation Degree in healthcare
