Job overview
An exciting opportunity has arisen for a knowledgeable, motivated Discharge Keyworker to join the Stroke team at Bournemouth Hospital.
This post offers the successful individual the opportunity to enhance their discharge knowledge and practically support both patients and staff ensuring that patients experience a safe discharge.
The successful individual will spend the majority of their working day with the MDT and liaising with patients, relatives and the wider discharge network to streamline complex discharges:
You would be expected to:
• Support the ward MDT with assessment and decision-making.
• Liaise between the ward staff, therapists, social workers and external partner’s staff.
• Guide ward staff to improve discharge planning referrals/ process.
• Support effective discharge planning reporting via the discharge related IT systems.
• Complete appropriate referrals and discharge paperwork
• Booking joint therapy and family meetings.
This role is part time, 22.5 hours per week, set days: Wednesday, Thursday & Fridays.
Base Location: Royal Bournemouth Hospital

Essential criteria
- Ability to work under pressure, individually and within a team
- Able to prioritise own caseload
Knowledge
Essential criteria
- Knowledge complex discharge planning process.
- Understanding of assessing and problem solving issues related to discharge planning
- Understanding of multidisciplinary assessment and treatment for older people / vulnerable adults within acute care and community settings.
Qualifications
Essential criteria
- Good standard of general education
- NVQ level 3: health / social care management or equivalent
Technical Skills Competencies
Desirable criteria
- Use of IT clinical systems ie: e-CaMis Ena, discharge databases
Experience
Essential criteria
- Experience of working in a hospital setting
- Understanding of complex discharge planning in care of the elderly, stroke, long term conditions and vulnerable adults.
Desirable criteria
- Previous experience of working with patients requiring CHC and Social Services involvement.
