Job overview

An exciting opportunity has become available to join the team in the new Frailty SDEC service at Dorset County Hospital.  We are expanding our acute frailty service, and the post holder will work as part of the multidisciplinary team in assessing patients who present in the Emergency Department as well as planned frailty SDEC admissions.  Frailty SDEC will be located in the Portesham Unit which has been designed to meet the needs of this client group.

Frailty SDEC will commence as a Monday - Friday service with  plans to expand  to  a seven day service. 

 

Main duties of the job

The key objective of Frailty SDEC is to provide safe, effective high-quality care for our older population in Dorset.  Assessment, investigations and treatments will be provided on the same day, leading to a safe co-ordinated discharge from the department.  Liaison and networking both with staff in the Emergency Department and the local community services will be key to co-ordinating the care of our patients

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • Educated to GCSE Maths and English grade C or equivalent
  • Care Certificate
  • NVQ Level 3 or equivalent in Health Care

KNOWLEDGE & EXPERIENCE

Essential criteria
  • Relevant experience working in an acute medical or surgical ward and with older people
  • Evidence of an ability to practice autonomously and with little supervision in a clinical environment whilst remaining within Trust policy
  • Experience and evidence of excellent customer care skills
  • Evidence of an ability to supervise a team
  • Understanding of the role of the Senior Health Care Assistant/therapy assistant

SKILLS & ABILITIES

Essential criteria
  • Evidence of ability to maintain effective working relationships with colleagues, patients and visitors
  • Evidence of good communication skills
  • Demonstrate an understanding of confidentiality
  • Evidence of an ability to respond to changing priorities, organising workload to meet deadlines
  • Competency in Venepuncture, Cannulation and ECG recording
  • Evidence of ability to produce accurate and legible documentation
Desirable criteria
  • Ability to work with hospital IT systems such as, PAS, ICE, Microsoft programmes
  • Evidence of an ability to work within the multidisciplinary team
