Are you looking to be part of an experienced team that have recently been awarded with significant investment? Our Dorset Stroke and Neuro Service would love to hear from you where ideas are shared, and visions are supported.

Our integrated community stroke and neuro team is AHP lead and will provide various pathways including supported self-management, advice with signposting, intensive rehabilitation (ESD), complex management and reviews. You will be working in a multidisciplinary team including social work, neuropsychology, rotations across the acute and community pathway and opportunities for advanced practice. You will have access to a wide range of specialists who can help to increase your opportunities for learning and development of skills.

This is a 12 month fixed term post of 37.5 hours based at Forston Clinic. Hours can be met flexibly with consideration of condensed hours. Our service is a 6-day service and we promote hybrid working with mixed caseload of community, base and working from home;

North Dorset (base at Yeatman Hospital) covering Shaftesbury, Gillingham, Sherborne and Blandford.

West Dorset (base at Forston Clinic) covering Weymouth & Portland, Lyme Regis, Purbecks and Dorchester.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only.  Please obtain permission to be released on a secondment before submitting an application.

We have recently been awarded significant investment in Dorset to develop Stroke and Neuro Services across Dorset to meet the National Stroke Service model published by NHS England last year (particularly with regards to Integrated Community Stroke Services- ICSS). 

Under the supervision of a registered Professional, the post holder will be expected to:

 Be responsible for a specific caseload  providing advice, support and signposting 

To work effectively as a member of the Stroke and Neuro Service 

Provide a flexible, client centred intervention providing personalised information and support to patients and carers/family on the Dorset Stroke and Neuro service caseload 

To work with people to identify goals and contribute to the care planning process

To link patients /carers with various voluntary sector organisations including Stroke Association and use of digital support platforms

Navigating and assisting referrals to health and well-being resources/services – Live Well Dorset, Social prescribers, leisure sector and support groups

To contribute to 7 day working 

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
