Job overview

We have an  exciting opportunity to join our community specialist palliative care service as a community clinical nurse specialist (band 6) primarily covering the geographical area served by Forest Holme Hospice (Poole).  

Applications are invited from  registered nurses who can demonstrate significant experience of working within palliative care (or other relevant field of practice) who wish to develop their skills and practice within specialist palliative care.   

Base Location: Forest Holme Hospice

Interview Date: TBC 

Main duties of the job

  • To provide specialist palliative Care advice, intervention and education to patients, families and healthcare professionals through multidisciplinary team working in the community setting.
  • To provide specialist nursing expertise, facilitation of evidence-based, patient-centred care for a defined caseload of patients who have complex needs.
  • To provide information, specialist advice in symptom control and psychological support to patients with cancer and non-malignant life limiting diseases and their families.

Experience

Essential criteria
  • Demonstrable experience of working within the Palliative Care speciality at band 5 or above – hospital or community. Managing own workload effectively
  • Teaching, supervising and mentoring staff
  • Experience of multi-professional working.

Qualifications

Essential criteria
  • Registered Nurse with NMC
  • Palliative Care qualification
Desirable criteria
  • Advanced communication skills training
  • Recognised teaching qualification
  • Ability to prescribe medication within the remit of their role

Skills

Essential criteria
  • Computer literacy within a range of applications
  • Knowledge of EPR
  • Relevant clinical skills
  • Effective written, IT and verbal skills

Knowledge

Essential criteria
  • Broad clinical knowledge of speciality
  • National clinical guidelines and service frameworks relevant to palliative care
  • Knowledge of governance and palliative care guidelines
