The Community Support Worker team are looking for a new Community Support Worker to join their friendly team, who has patient care at the heart of all we do! We are looking for dynamic and adaptable individuals, travelling in North Dorset's countryside providing a high standard of care to people in their homes. You will need to be an excellent communicator and enjoy working as part of a team, as well as competent with lone working.

Working as part of an integrated, multi-disciplinary locality team,  proactively and reactively providing domiciliary support, including palliative and end of life care, in patient’s homes. With your Work Base being Westminster Community Hospital in Shaftesbury, we also visit patients in Blandford and Sherborne. 

To contribute to the aims and objectives of the service by implementing and undertaking appropriate methods to:

- Prevent avoidable hospital admissions                                                       

- Support early supported discharges from Acute and Community    Hospitals

- Increase independence

- Reduce dependence on long-term health and/or social care services

To participate in the assessment of identifying patient’s needs, record observations and outcomes of interventions, and communicating feedback to colleagues involved.

Please note although this position is community based in North Dorset, your work base will be Westminster Memorial Hospital, Shaftesbury.

To provide the following domiciliary support at a high standard, with respect and dignity, whilst keeping the aims and objectives at the forefront:

  • To assist with all aspects of personal hygiene, including toileting needs.
  • Support patients with mobility ensuring the relief of pressure areas are appropriately managed.
  • Encourage healthy skin care and carry out regular checks to pressure areas and minor wounds.
  • Food preparation and serving of meals, with the assistance of feeding if required. Ensuring the patient’s fluid and food intake levels are appropriate, along with sufficient food supplies in the property.
  • Conducting environmental checks, ensuring cleanliness and temperature in the patient’s home are suitable, whilst ensuring the patient’s bed is clean and ready for use. Providing advice, guidance and signposting to appropriate services to patient and/or their families regarding how to maintain care, independence, promote health and wellbeing between visits.
  • Highlighting any concerns in relation to any of the above points to the relevant team(s).

  • Excellent communication, interpersonal, organisational and decision-making skills

  • Demonstrate skills in written and spoken English, adequate to enable the post holder to carry out the role effectively

Job Specific Experience
