Come and join our Dorset Stroke and Neuro Service serving the West and North of Dorset.  We have recently been awarded significant investment in Dorset to develop Stroke and Neuro Services across Dorset to meet the National Stroke Service model published by NHS England last year (particularly with regards to Integrated Community Stroke Services- ICSS).  This is a really exciting time as we significantly expand our existing services and embed innovative ways of working to improve clinical outcomes for patients following stroke or neurological illness or injury.  Importantly this once in a generation investment increases our team threefold and significantly extends the time patients can access the service moving to a needs led, rather than time limited service.  Our integrated community stroke and neuro team is AHP lead and will provide various pathways of care including supported self management, advice and signposting, intensive rehabilitation (ESD), complex management and reviews.

This role is for a team & SSNAP data administrator -  full time. 

To support the Administration functions of the integrated community stroke and neuro service ensuring that the administrative needs of the services are achieved to the required standards.

To drive the delivery of efficient and accurate processing in the management of the patient journey from triage, treatment, onward referral, typing of reports/letters to discharge.

To establish and maintain communication with key stakeholders for the services – Primary Care referrers, clinicians, secondary care and independent providers.

To ensure patients and all contacts to the service are dealt with in a friendly and professional manner and that the experience is positive.

Ensure patient pathways meet the Triage target and 24/72 hour Referral to Treatment target (RTT).

To provide administrative support to the senior managerial and clinical leads of the service.

Dorset HealthCare is in partnership with The Prince’s Trust to support people getting into work, as part of this partnership, we offer an employability service to support individuals between 16-30 years old with application writing, interview skills and other employment support. Our team expansion includes nursing, social work, neuropsychology and voluntary sector roles, as well as apprentices, rotations across the acute and community pathway and opportunities for advanced practice.  Over coming years we aim to further bring together a range of specialists working across the fields of stroke and neurology in Dorset, meaning our integrated team will have access to a wide range of specialists with many years of experience, many of whom work within a national capacity within their field to increase opportunities for learning and development of skills as well as opportunities for involvement in research.  We have a programme initiated within Dorset developing communities of practice regarding psychological care and motor relearning, which postholders can join.
