With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

To work as part of the Community Heart Failure Service under the direction of the Specialist Heart Failure Nurses

Participate in the delivery of patient care, by ongoing assessment, monitoring and education to optimise the management of people with diagnosed heart failure to improve patients’ quality of life.

The post holder, will visit patients in their home, arrange bloods, record blood pressure, pulse, ECG and also recording weight and fluid intake records as directed or requested.

Will carry out phlebotomy when required and requested and carry out administration tasks as necessary.

To provide the highest standard of safe, effective and compassionate care through the effective use of evidence based practice

To support a continuously improving service, in line with the clinical governance framework, Trust objectives, and the NHS performance framework.

To continually develop professionally to ensure the post holder has the appropriate level of skill, knowledge and competence to meet service/specialty requirements.

Applies effective communication in clinical practice with a caring and compassionate attitude at all times.

Establishes and maintains effective communication with individuals, the wider nursing team, and other health professional regarding patient’s conditions, care planning and treatments.

Be part of the nursing team to ensure effective communication with patient’s relatives and carers, explaining medical conditions, progress and investigations taking into consideration level of understanding, culture and background.

Develop and maintain effective channels of communication within the team, engaging with staff at all levels.

Recognise and actively support barriers to communication to ensure positives outcomes and escalate appropriately to address.

Communicates regularly and effectively with the MDT and stakeholders regarding all aspects of clinical care and service delivery, including environmental issues.

Must be able to demonstrate the English language proficiency level required for the post

Recognise and escalate concerns relating to vulnerable patients adhering to trust safeguarding procedures.

Be able to initiate or respond to emergency situations.
