With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

Prif ddyletswyddau'r swydd

We are excited to offer an opportunity for a band 3 member of staff to join the Community Integrated Assessment Team, to work within the Integrated Intermediate Care team. The post is a permanent position and will be a full-time post.

The successful candidate will be expected to work as part of a multidisciplinary community based team, assisting with the delivery of therapy and rehabilitation to those patients who are referred to the Community Therapy Team.

As this is a Trustwide post, the successful candidate may be required to work between the Stockton and Hartlepool localities, but will be predominantly based in the Stockton locality.

You will be expected to work flexibly and available for both extended day and seven day working. In return we will provide you with robust induction support and the opportunity to develop with the guidance of experienced Senior staff. We actively support CPD. We pride ourselves on the friendliness of our service.

Qualifications

  • NVQ Level 3 or alternative relevant qualification/experience
Meini prawf dymunol
  • Additional training such as short courses to diploma level or equivalent.
  • Certificate in Health Care Studies, Foundation Degree or relevant qualification.

Experience

  • Clinical Experience of working in Therapies
  • Demonstrate sound basic knowledge and understanding of a broad range of medical conditions/pathologies, particularly those affecting older people
  • Experience of working in a multidisciplinary team

  • Experience of prescribing and fitting less complex equipment /adaptations required to meet peoples needs under the supervision of a therapist
  • Experience of working within a rehabilitation setting.
  • Experience of working within a community setting.
  • Experience of using Systm1

Skills

  • Able to work autonomously with arms length supervision, eg to deliver services to people in their own homes whose needs require less complex interventions.
  • Able to prescribe and fit less complex equipment/adoptions required to meet peoples needs under the supervision of a therapist.
