A new and exciting opportunity has arisen for 2 Community Mental Health Practitioners to work within Primary Care Networks (PCN) and GP practices and secondary care (Ripon Integrated Community Team). 

The focus or partner agencies is on prevention and personalisation of care, whilst working together to make the best use of shared resources.

The successful post-holder will work in partnership with practice colleagues and the secondary care integrated community team in joining an ambitious locality network that is committed to developing a strong, caring, and compassionate multidisciplinary workforce.

As a valued team member, your role will involve offering specialist mental health advice for people with a range of mental health needs, ensuring that those with mental illness do not fall between gaps in services.

You will also contribute to safe and effective transitions in care between Adult Mental Health services. You will improve access, undertaking triage, assessments, brief interventions, and reviews within primary and secondary care. As this is a newly developed role aligned to transforming mental health services, you will have a key role working into the new community hubs as they develop. You will contribute to networking and navigation expertise, enabling access to a range of appropriate mental health support across the community system.

The post holder will be an effective member of the multi-disciplinary team. They will be professionally accountable and responsible for patient care, undertaking initial assessment with signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in e.g. GP surgeries, patient’s homes and other care settings. This will include providing clinical advice to GPs such as specialist opinion.  

In line with the principles of community mental health transformation you will develop links and foster a culture of ‘partnership’ between Primary Care Teams Voluntary Care Sector and Mental Health Services.

If you are a qualified Occupational Therapist, Mental Health Nurse or Social Worker, with a determination for supporting local people with mental health problems and their families, and are looking for an exceptional opportunity to contribute to the development of an innovative and new way of working - then we look forward to hearing from you!  

You will be expected to be on the duty responder rota and work closely with all members of the MDT.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Ripon and Masham Primary Care Network comprises of 4 practices, North House, Ripon Spa, Dr Akester & Partners and Dr Ingram and Partners with 3 of these based in the small city of Ripon and 1 in the nearby market town of Masham. They support the mixed patient group across both the urban and rural settings and are a tightknit PCN with close working relationships between the practices including the shared pharmacy team, social prescribers, First contact physiotherapists and grown shared admin functions. 

We would welcome enthusiastic innovators who are willing to develop this role and make the service the very best it can be for local people. We want individuals able to work systemically across the local community system to support people holistically.

Qualifications

  • Current professional registration with an approved professional body.
  • Evidence of Continuing Professional Development
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
  • 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

  • Experience and evidence of mental health triage, assessment and supporting connecting people across service providers
