Job overview

An exciting and rewarding opportunity has arisen, for a band 6 community clinical lead nurse. The ALD South Tees Community Team provides services across both Middlesbrough and Redcar and Cleveland with a base at Flatts Lane Centre and is  made up of  Nursing, Psychiatry, Psychology and  AHP's (including Physiotherapy, Occupational Therapy, Speech and Language Therapy) staff.   

The team work collectively and collaboratively with others to ensure a  high standard of care is provided, and are always willing to go above and beyond what is expected when required, and to ensure people with a diagnosed learning disability and their families and carer are supported by delivering high quality clinical care. 

We work predominantly 9-5 but there is an expectation to take on evening and weekend working plus on call duties

The post holder will work as part of the Multi Disciplinary Community Learning Disability Team, work towards addressing and supporting the  needs of adults with a learning disability who have complex physical and emotional health needs. 

The focus of the team is to provide an alternative to in-patient care in-line with the transforming care agenda, focusing on people with a learning disability who are experiencing (but not exclusively) mental health difficulties, behavioural challenges, placement breakdown and medication titration who should be treated in the least restrictive environment with the minimum of disruption to their lives. 

 

Main duties of the job

The successful candidate will need to have a genuine desire to improve the mental health and well-being of adults with learning disability and behaviours that challenge. They will work as part of a  MDT delivering high quality secondary care and enhancing the community services, working with individuals  who require specialist interventions  using trauma informed care & a recovery-based approach.

The successful candidate must be motivated, flexible,  able to communicate effectively,  will be expected to  be able to travel independently to various locations within the Trust on a regular basis.

A trust car lease scheme is available. 

The successful candidate will:

  • Hold a complex caseload, completing assessments, delivering treatment and interventions, monitoring and evaluating outcomes, implementing Clinical Care Pathways.
  • Provide clinical supervision, support, advice and guidance to junior members of staff, students, colleagues and others.
  • Maintain good communication, documentation, providing reports and information within agreed timescales.
  • To adhere to legal frameworks/ policies and procedures e.g. Safeguarding, Mental Capacity Act and Mental Health Act
  • Maintain their NMC Registration/ PIN ensuring the compliance with the NMC Code/ revalidation.

Detailed job description and main responsibilities

The post holder will deliver a quality service and ensure the individual is at the centre of their care ensuring they are fully involved in all aspects of assessment, planning and care and treatment as appropriate, including being professionally and legally accountable.

The service is looking for RNMH or RNLD with relevant LD experience or who have a willingness to develop, staff must be able to care coordinate and should be able to work well independently as was as part of a wider team.

The team will cover a 7-day service, enhanced hours , covering the whole geographical patch.  Will be responsible for managing a clinical caseload under the supervision of the Professional Lead, providing assessment, advice and support to clients, carers and other professionals and delivering training.

We are looking for people who are enthusiastic and who are willing to help support, develop and shape the team.

Person specification

Qualifications and Experience

Essential criteria
  • Current professional registration with Nursing & Midwifery Council. RMNH/RNLD
  • Evidence of Continuing Professional Development.
  • Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months
  • Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 month
  • Minimum of 12 months experience post completion of preceptorship experience working with people with mental ill health, Autism or Learning Disability in a care environment.
  • Working collaboratively with service users and their families/carers
  • Providing clinical supervision to individuals or groups as professionally appropriate.
  • Supervising or assessing students on practice placement.
  • Working in a multi-disciplinary team
  • Quality improvement activities

Knowledge

Essential criteria
  • Demonstrable knowledge to post-graduate level of evidencebased practice in caring for patients in the designated field
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Detailed understanding of Safeguarding and its application in practice

Skills

Essential criteria
  • Experienced in communication skills working with a person with a learning disability
  • Experienced in MDT working
