With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

Post holder will be the first point of contact for all service users including carers and GPs, making appointments and dealing with screening queries. Ensuring the effective communication and efficient running of the administrative pathway of the screening programme. To book screening appointments for patients including management of clinics. To monitor all patient and GP communications generated from the screening system and deal with onward referrals into Ophthalmology following failsafe procedures.
Work with the failsafe officer to implement failsafe procedures including already established pathways but contribute to new and updated guidance. Monitor all patients from invitation to referral as they move through the screening pathway and updating all databases used to monitor breaches.
Build on already established relationships within both referral centres to ensure smooth exchange of data to enable update of pathways
Assist in the collation and production of statistical information using the reporting systems within the screening software
Main duties of the job
- Able to identify barriers to screening using reports and a multitude of screening reports
- Identify any trends in demographic uptake
- Use own judgement when communicating with service users who require encouragement to engage
- Monitor referral timeline and act appropriately when breached
- Respond to frequent enquiries from patients, carers and all other stakeholders using written and verbal communication using empathy, sensitivity and own judgement skills
- Deal with all queries in a professional and confidential manner, especially where there are barriers to understanding.
- Able to present data using Microsoft office tools
- Monitor all referral pathways and request feedback data from both referral centres using failsafe reporting tools within the screening software
- Ensure no patient is lost to follow up in hospital eye services
- Be point of contact for all service users
- manage appointment system, ensure clinic capacity and deal with any queries daily
Update patient notes accurately and work with Failsafe Officers to action any breaches.

Detailed job description and main responsibilities
- Communicate effectively at all levels including Senior Management, Heads of Service, other departments, service users and external stakeholders.
- Liaise internally with colleagues to ensure the smooth running of the admin/failsafe function
- Be first point of contact for all service users, queries/booking appointments/changes to appointments.
- Encourage and re-assure patients the importance of screening
- Regular requirement to deal with sensitive and confidential information demonstrating empathy, tact and diplomacy
- Requirement to communicate effectively with people from different cultural backgrounds.
- Maintain confidentiality of patient information to which the post holder has access, including patient records when filing and using database systems.
Attend departmental meetings and participate in MDT providing feedback to the clinical
- Attention to detail, organisation skills and precise and clear communication to all service users and stakeholders
- Confidentiality to be maintained at all times
- Monitor own time management to ensure all duties are carried out
- Maintain screening database using GP2DRS
- Ensure timely registration of all new notification of diabetics from incoming e-mails ad-hock, to ensure national requirements are met
- Assist in the maintenance of the service database and other related information systems using appropriate technologies
- Use own initiative without direct supervision but with the daily support and leadership of the Failsafe officers
- Expectation to be proactive and have a flexible approach, as this post requires a degree of initiative and flexibility
- Good administration skills and close attention to detail and the ability to produce work consistently to a high standard
