Job overview

The Community Neurological Rehab Team (CNRT) based in Chichester and covering the West side of the Sussex area is seeking to employ a physiotherapist who is  enthusiastic about supporting people with neurological diagnoses to achieve their maximum potential.

This may include, but is not limited to, neurological conditions such as Stroke, Parkinson’s Disease, Multiple Sclerosis (MS), and Motor Neurone Disease (MND). We work closely with our Acute Hospital partners to deliver a responsive stroke pathway, and Specialist Practitioners to support those with long term neurological conditions. We also liaise with other teams in Sussex Community NHS Foundation Trust as well as external agencies such as West Sussex County Council, care agencies and care homes and charities such as the Stroke Association and Workability. 

This is a rewarding role which at times may be challenging, but you will be fully supported by our small friendly Multi-disciplinary Team. We consist of physiotherapists, occupational therapists, a speech and language therapist, stroke specialist nurses, MS practitioners, an MND practitioner, a psychologist, therapy practitioners, therapy assistants and our invaluable caseload coordinator and MS administrator.  We will support you with your career goals through ongoing development opportunities and are passionate about providing excellent evidence based care.

Main duties of the job

  • Complete a holistic neurological and functional assessments of people with a neurological diagnosis in their own home or in a clinic environment.
  • Assess for and prescribe equipment for the home environment and specialist equipment for rehabilitation needs as indicated.
  • Use a personalised care approach to set short and long term goals, and create collaborative rehabilitation plans for people that access our service.
  • Work autonomously, and as part of an MDT.
  • Supervise and support Therapy Practitioners and Therapy Assistants within the team.
  • Role model Sussex Community NHS Foundation Trust Values in interactions with people accessing the service and our MDT colleagues. 
  • Attend team meetings and participate in the development of yourself and other team members to ensure the delivery of best practice through  evidenced based care.
  • Support students on placement and liaise with local universities.

Qualifications

Essential criteria
  • Degree/ Diploma or equivalent HPC Registration
  • Evidence of CPD

Experience

Essential criteria
  • Experience of working as a Band 5 or above in a neurological and/ or community setting for at least 18 months
  • Broad range of experience in community and acute settings.

Skills

Essential criteria
  • Ability to use and analyse standardised assessments
  • Competent ability to assess and treat a wide range of complex conditions within neurological field.
  • Advanced Manual Handing skills. Knowledge of basic and complex equipment used to facilitate independence.
  • Broad base of general and neurological clinical knowledge. Good use of evidence based practice/ best practice.
