The Conquest Hospital in Hastings and our Community Services were rated ‘Outstanding’ and Eastbourne DGH was rated ‘Good’ Our two acute hospitals have Emergency Departments and provide 24 hour a day care, offering a comprehensive range of surgical, medical, outpatient and maternity services, supported by a full range of diagnostic and therapy services.

At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.
In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

Come and join our friendly, sociable, valued and experienced team working within an integrated Health and Social Care community rehabilitation service based in Eastbourne, covering the Eastbourne Hailsham & Seaford area.
The JCR service is a short-term service which assesses and treats people within their own home. The service aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life.

Main duties of the job
You will be part of a friendly, supportive, professional integrated team which provides a 7-day service and uses an inter-professional model of working. Continuing Professional Development is supported and supervision and training are integral to the working practices of the team. You will key work non-complex clients, provide rehabilitation to people with a variety of conditions, and respond where appropriate to urgent referrals supporting clients to stay in their own home.
Working for our organisation
The post holder will be required to work flexibly and visit people in their own home and liaise with clients in acute and intermediate care settings if necessary prior to discharge. The ability to travel around the county in a timely fashion is essential.

Qualifications
Essential criteria
- GCSE grade 4 or GCE C and above
- Level 2 functional skills or health care support worker apprenticeship / NVQ level 2 in relevant subject
- Level 3 apprenticeship for example senior health care support worker or similar and / or NVQ level 3 in relevant subject or equivalent level of knowledge and experience
Experience
Essential criteria
- Experience in health or social care settings such as employment, work experience volunteering or placements
Desirable criteria
- Demonstrate the values of the NHS and commitment to personal and professional development
Skills
Essential criteria
- Demonstrating respect, kindness and empathy at all times
- Understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks
- Able to understand and follow procedures protocols and plans
