Trosolwg o'r swydd

Are you looking for a new challenge? Are you an experienced registered nurse with a good understanding of the discharge pathways? Are you passionate around helping patients/carers/families to be informed around discharge options to them? Then this role could be for you!

Our current Discharge Hub model on both Acute Sites - Eastbourne Hospital and the Conquest Hospital in Hastings is changing. We are forming a new Transfer of Care Hub (TOCH) which will be an integrated model working with key stakeholders including the Divisional Wards, Social Care, Voluntary Services, Housing, Homeless Services, and of course our patients/carers and families who use this service.

The TOCH will work 7 days a week so weekend work/bank holidays will be expected.

We have 2 TOCH's - one at the Conquest Hospital and one at the Eastbourne Hospital, so cross site work will be expected. We are also an integrated trust, so occasional working in other sites such as Bexhill or Rye Hospital may be required.
