Our extensive health services are provided by over 7,000 dedicated members of staff working from two acute hospitals in Hastings and Eastbourne, three community hospitals in Bexhill, Rye and Uckfield, over 100 community sites across East Sussex, and in people’s own homes. 


In 2020 the Care Quality Commission (CQC) rated us as ‘Good’ overall, and ‘Outstanding’ for being Caring and Effective. The Conquest Hospital in Hastings and our Community Services were rated ‘Outstanding’ and Eastbourne DGH was rated ‘Good’ Our two acute hospitals have Emergency Departments and provide 24 hour a day care, offering a comprehensive range of surgical, medical, outpatient and maternity services, supported by a full range of diagnostic and therapy services. 


In the summer of 2020, the Trust launched its ambitious ‘Building for our Future’ (BFF) programme. This once in a lifetime programme aims to redevelop and expand our hospitals at Eastbourne, Hastings and Bexhill, transforming the environment in which we provide care for generations to come.

Our centre for trauma services and obstetrics is at Conquest Hospital, while our centre for urology and stroke services is at Eastbourne DGH. At Bexhill Hospital we offer a range of outpatient, day surgery, rehabilitation and intermediate care services. At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.

In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

Are you looking for a new challenge? Are you an experienced registered nurse with a good understanding of the discharge pathways? Are you passionate around helping patients/carers/families to be informed around discharge options to them? Then this role could be for you! 

Our current Discharge Hub model on both Acute Sites - Eastbourne Hospital and the Conquest Hospital in Hastings is changing. We are forming a new Transfer of Care Hub (TOCH) which will be an integrated model working with key stakeholders including the Divisional Wards, Social Care, Voluntary Services, Housing, Homeless Services, and of course our patients/carers and families who use this service. 

The TOCH will work 7 days a week so weekend work/bank holidays will be expected.

Main duties of the job

  • Lead on the discharge of patients within the service.
  • Participate in the education, development, and supervision of other staff members within the TOCH.
  • Support and educate the staff within the wards and divisions around the discharge process/pathways and support complex case management.
  • Act as a role model for staff.
  • Liaise with the multidisciplinary staff to meet the needs of the patients and carers/families. i.e. Local Authority/Housing/VCSE/CHC/ICS
  • Liaise with partner and external organisations to achieve discharge outcomes.
  • Act as the patients advocate with regard to discharge planning.
  • Maintaining contact with patient’s, carers, and relatives in relation to discharge planning and supporting conversations.
  • Assess, plan, implement and evaluate evidence-based practice.
  • Facilitate safe and timely discharge from hospital.
  • Maintaining and updating Electronic Patient Record systems in relation to Discharge Ready status and discharge plans.
  • Completing complex assessments, i.e., Continuing Healthcare (Fast Tracks), and Trusted Nurse Assessor forms.
  • Enhanced knowledge around Mental Capacity Assessment and Best Interest Decisions.
