At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.
In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.
Job overview
This is an exciting opportunity to join our Integrated Discharge Team (IDT) Physiotherapy Team Lead to further develop our service at Conquest Hospital.
The Integrated Discharge Team work closely alongside Occupational Therapists, Nursing staff, Adult Social Care, our community colleagues and the Transfer of Care Hub in order to assess, treat and provide definitive discharge plans in collaboration with our patients and their next of kin.
ESHT is a fully integrated Trust providing therapy services within our two acute sites, three intermediate rehabilitation units and throughout the community across East Sussex.
We are looking for someone with proven recent experience of working with a variety of complex patient conditions; including frailty, patients diagnosed with dementia or those who exhibit memory impairment, palliative/end of life care and the initial assessment of neurological conditions.
Main duties of the job
You will jointly lead the induction and support of new graduates to ensure their competence in the assessment and treatment of our acute medical patients. You will work collaboratively with our existing Team Lead to support the entire IDT Physiotherapy team, including the completion of appraisals, actively participate in training to develop their clinical reasoning and the enhancement of their knowledge and skills.
You will need the skills to risk assess and make appropriate discharge plans, have an awareness of current professional and research practice and be willing to participate in national and local audits. Well-developed communication and interpersonal skills are essential as well as excellent organisational and time management skills.
You will need to show your commitment, enthusiasm and drive to develop your own clinical, managerial and team working skills within the service. You must be able to communicate effectively at all levels, work autonomously and be able to demonstrate effective teamwork. You will be supported through regular meetings, 1:1 sessions, ongoing education and development. All team and clinical physiotherapy leads across all specialities are also welcomed to participate in our quarterly Professional Leads forum.

Qualifications
Essential criteria
- A BSc in Physiotherapy/post grad MSc Physiotherapy
- Health Professions Council Registered
- Evidence of relevant postgraduate development
- Evidence of comprehensive CPD to Masters level or equivalent experience, knowledge and skills.
- Postgraduate: higher level learning. MSc modules/PG certs/diplomas in clinically relevant specialist area
Desirable criteria
- Leadership qualification/ training
Experience
Essential criteria
- 3 years as a Band 6 Physiotherapist or equivalent experience
- 18 months acute hospital discharge planning
- Experience of developing and appraising staff
- Delivering services along evidence based guidelines
- Experience leading student placements, and scoring performance
Desirable criteria
- Previous Experience as a Senior Physiotherapist
- Experience of Service Development
- Experience of performance management
- Experience within acute and community settings
Skills
