Job overview
The Community Health Care assistant must be able to work automatously and as part of the multi-disciplinary team within the Bridging service to provide a seamless service & highquality care to patients. The Community Health Care assistant is expected to undertake care to meet the needs of the patients under the supervision of an appropriate Registered
Health Care Professional, working within the Trust’s policies and procedures. To visit patients within their own home in the community, working to care plans, protocols and polices, whilst reflecting individual choice and maximising patients’ independence and quality of life.
It is expected that the Health Care Assistant will continue to develop their own capabilities and knowledge of clinical tasks in line with the Trust’s career pathway
Main duties of the job
MAIN DUTIES & RESPONSIBILITIES
- At all times maintain the dignity and respect of patients
- To provide planned advice, support and instruction to patients and where relevant their carers and other professionals, to promote understanding of the aims of their treatment plan / goals and to ensure consistent approach to care
- To provide domiciliary care for patients where they are unable to, encourage patients to reach there optimum.
- Maintain excellent communication with patients, relatives and members of the MDT regarding all aspects of care demonstrating a variety of communications skills in accordance with the client group
- To undertake manual handling and therapeutic handling techniques with patients both single handed and as part of a team
