Job overview

We are looking for a dynamic individual who will provide specialist Occupational Therapy assessments for Frailty patients attending Accident and Emergency (A&E), Acute Medical Units (AMU), the Frailty Hot clinic, Frailty Assessment Unit (FAU) & ward. The aim will be to ensure all patients receive specialist review, as early as possible, and that resources are deployed to avoid admission or reduce length of stay.

Main duties of the job

  • You will have significant experience working effectively at Band 6/7 level in an NHS acute setting, or equivalent, working with older adults with a variety of conditions. They will have highly specialist current knowledge and skills related to the management of complex conditions within the specialty
  • You will have a dynamic approach to work and be capable of leading a team, including in times of change.  They will be expected  to manage the daily running of the team to ensure the delivery of high quality patient-centred care within the specialty.
  • This role has a strong clinical focus: You will use advanced clinical reasoning skills to provide specialist support and advice to the integrated therapy team (occupational therapy and physiotherapy) whilst maintaining a clinical caseload of  complex patients as an autonomous practitioner.
  • You  will take part in the weekend and Bank Holiday rota. 
  • Work as an advanced autonomous practitioner:  Assesses, sets goals, provides treatment and progresses own patient caseload which contains patients with complex and specialist needs.
  • Lead in specialist area; contributes to development of specialist services and relevant policies helping implement policy changes and monitoring standards of clinical practice.
  • Supervise, teach, and evaluate students and other staff working in own specialist area.

  • To undertake an advanced assessment of  patients in the Emergency department, ambulatory areas and on the general medical ward as an autonomous practitioner to determine the appropriate interventions and plan for effective safe discharge 
  • To maintain records in line with HCPC standards
  • To work with other Team Lead therapists within the team to ensure that non clinical duties including rotas,  time to connect, appraisal, student placements, in service training, and probation are effectively and efficiently managed to ensure smooth running of the service
  • To be responsible for planning, prioritising and managing a caseload of patients in the inpatient setting using advanced evidence based/client centered principles to assess, plan, implement and evaluate interventions. As well as assisting less experienced staff in maintaining their own caseload.
  •  To be accountable for all aspects of the professional clinical work.
  •  To ensure that the Frailty Therapy Lead is made aware of any changes/ challenges to the service to ensure quality and effectiveness is maintained.
  • To work with the Therapy Lead to deliver service improvement based on evidence, audit and service need
  • To take and active part/ lead in the training of the therapy team and other health care professionals when appropriate
  • To engage in relevant audit

Qualifications

Essential criteria
  • HCPC registeration
  • Degree/Diploma in Occupational Therapy
Desirable criteria
  • RCOT membership

Experience

Essential criteria
  • Experience in acute inpatient general medicine band 6 or equivalent
  • Evidence of relevant post graduate learning and development
Desirable criteria
  • Evidence of service development/ audit

Experience

Essential criteria
  • significant time at Band 6 level or above working in Frailty or ambulatory areas
Desirable criteria
  • Experience of teaching to MDT groups

NHS Experience

Essential criteria
  • Minimum 6 months NHS Experience as an occupational therapist
