Job overview
There are two posts available for the role of Band 6 Community Psychiatric Nurse.
The main duties of the role include, but are not limited to: Assessment and Care Co-ordination of clients experiencing a range of mental health conditions including but not limited to, Schizophrenia, Bi-Polar Disorder, Personality Disorders, Depression, Anxiety and OCD.
As well as this the team runs a both Depot and Clozapine Clinics, and the person would be expected to cover these as and when required, or potentially take this on as a regular part of their working week.
The role covers a wide geographical area within Mid-Essex, as such a driving license and access to a car is essential for the role. The team is a made up of other CPN’s, Social Workers, Occupational Therapists, Psychologists and Psychiatrists as well as Support Workers and admin staff.
The team hold regular MDT and Business meetings to discuss the ongoing care of the clients as well as the functioning of the team.
Main duties of the job
Band 6 Care Coordinator role within The Gables Specialist Mental Health Team, involving caseload management, duty service, depot clinics and clozapine clinics. EPUT are contracted to carry out duties under the Care Act 2014 and so the role also includes: investigation of safeguarding inquiries, completing care act assessments as well as applications for funding from the 117 or social care funding panels
