Job overview

An exciting opportunity has arisen for a full-time position as an Assistant Practitioner role within the Community at the Front Door IRAS Team.
 
This is an excellent opportunity for you to work as part of an established admission avoidance team.
 
The Community at the Front Door Team is a multidisciplinary team that support a complex range of patients admitted to Accident and Emergency and the Urgent Treatment Centre seven days a week.
 

Main duties of the job

The role will involve you working closely with all elements of the community at the front door team and the emergency department. You will support safe discharges and provide additional services either by referral or signposting to prevent unnecessary admission and ensuring patients receive the right care in the right place under the direction of a registered nurse.

Experience in care, admission avoidance and working in a clinical setting would be advantageous.
