This exciting new Specialty Doctor post has been developed to provide consistent, high quality Medical input and clinical leadership to support our established Virtual Hospital (VH) and Urgent Community Response Services (UCR). We are looking for a friendly, enthusiastic, and innovative individual who will work collaboratively with the team to support service demands.  We are seeking individuals with a keen interest in acute care and frailty, who would like to develop their skills and expertise with a view to formal training in elderly medicine.

You should be eligible for full registration with the GMC at time of appointment and hold Section 12 (2) approval under the Mental Health Act 1983. You will also need to provide evidence of achievement of ST1/CT1 competences in psychiatry at time of application and likelihood of achieving CT2 competences in psychiatry by the start date of the post.

Main duties of the job

The Virtual Hospitals purpose is to deliver care to residents within the local population of West Essex in the adult’s home through a fully integrated community led Virtual Hospital that is capability driven and consultant led with community and acute wrap around service. The remit of the service is to deliver admission avoidance and early discharge. Introducing holistic support in their own home with remote technology available dependent on need. Providing enhanced clinical intervention and monitoring for higher acuity adults that would otherwise need an acute hospital admission or extended stay within acute bedded care, encouraging the adult to be involved in their own care and support their independence in the community. The Virtual Hospital is interdependent on Urgent Community Response services as a first response team.

The post holder will be expected to assist patients remotely in their own homes with nursing teams and ACPs.  He/she is also expected to support any in reach services in various surrounding acute hospitals (Princess Alexandra hospital, Harlow as the main site)

 The day to day duties include triaging of referrals from either telephone calls or emails and carrying out CGAs (Comprehensive Geriatric Assessment) with MDTs to devise a holistic management plan.

Detailed job description and main responsibilities

 

1.         Observe the Trust’s agreed policies and procedures.

2.         Provide, with consultant colleagues, a service in the specialty for the Trust with responsibility for the prevention, diagnosis and treatment of illness, and the proper functioning of the department.

3.         Provides support to other junior resident doctors in Continuing Medical Education session in the West Essex Community hospitals and also providing learning sessions to the Clinicians in the Hospital at Home service.

4.         Attend various service development programmes including providing presentations about our service to educate the community services including Primary Care Networks throughout West Essex Region and Secondary hospitals every year to encourage the services to identify and utilise our services as part admission avoidance.

5.         Provide input into monthly leadership meetings, performs quality improvement projects regularly with Consultant support and support colleagues to participate in QI projects.

3.         Honour out-of-hours responsibilities, including rota commitments where contracted.

4.         Provide agreed cover for colleague’s periods of leave.

5.         Participate in audit and continuing medical education.

6.         Contribute to the training of junior medical staff and medical students.

7.         Be involved in research and teaching where appropriate.

Person specification

Qualifications

Essential criteria
  • -Full registration with the GMC with a licence to practice in GIM.
  • -MBBS or equivalent experience in Geriatrics in Acute Medicine and working towards MRCP.

Clinical Experience

Essential criteria
  • -Ability to offer a specialist clinical opinion on the full range of acute respiratory/cardiology and GIM conditions.
  • -Ability to take full and independent responsibility for clinical care of patients, in both in-patient and out-patient settings.
  • -Experience in Clinical Audit and Clinical Governance and the benefits of how this improves the quality of care provided to patients and ensuring this is embedded in clinical practice.
Desirable criteria
  • -Established track record in clinical research
  • Further sub-speciality experience.

Management/Administrative experience

Essential criteria
  • -Evidence of working effectively as a part of a multidisciplinary team.
  • -Ability to initiate and lead service developments and comply with all mandatory national audit needs for the speciality.
  • -Willingness to be involved in departmental management.
  • -Evidence of initiating, progressing and completing audit.
  • -Direct and extensive working knowledge of the UK National Health Service.
Desirable criteria
  • -Completed basic training in management skills.
