Job overview

The Occupational Therapy department has an opportunity available for an enthusiastic and experienced Occupational Therapist to join our integrated Acute Inpatient Therapy team at PAH.

This is a specialist post within our Emergency Department (ED), Short Stay assessment wards and Frailty service. We pride ourselves on delivering a high-quality service in a fast-paced, challenging environment. We provide comprehensive assessments and treatment plans with the aim of preventing deconditioning, unnecessary admissions and facilitating timely discharge.

You will work collaboratively with key stakeholders to further develop innovative ways of working that align with the NHS 5 year plan, this includes participation in workstreams including virtual frailty clinic, involvement in the implementation of discharge home to assess model within the OT service and development of meaningful activities programme within the Frailty Assessment Unit.

You will work closely alongside other therapists within the integrated therapy model, developing further skills and knowledge. You will have the opportunity to support junior team members within formal and informal supervision. You will be expected to support students on clinical placement.

Main duties of the job

You will be responsible for providing a high standard of assessment, advice, treatment and evaluation of patients in the Emergency Department, Frailty Assessment Unit and Short Stay  Assessment Units. This may include patients with diverse presentations and complex physical and psychological conditions.

You will work as part of an MDT to support patient flow, working across ED and Frailty assessment unit using clinical reasoning to identify appropriate frailty patients and facilitating patient transfers where possible, this will be dependent on their individual presentation and needs. You will be responsible for a clinical caseload involving complex case management and the facilitation od challenging discharges.

You will also have the opportunity to build on your previous experience to further develop your clinical and managerial competencies to be part of a dynamic team to drive forward future developments.

It is required that you work as an autonomous professional according to the Royal College of Occupational Therapy Professional Conduct and Standards, and the Health Care Professions Council's regulations.

Detailed job description and main responsibilities

As a team we share in the vision ‘home first’, this approach acknowledges that often patients needs are best managed in their own environments, this is often also the wishes of the patient. We are key to identifying risks and barriers to this and finding solutions with the collaborative support of the multidisciplinary team. We have established links and regularly network with our primary care services, social services and external agencies within the ICS to make this process from hospital to home as streamline as possible.

You will be supported by our Occupational Therapy team leader with yearly appraisal and regular formal and informal supervision. In addition to attending and contributing to targeted IST within the ED and Frailty therapy team you will be invited to the established therapy department weekly in-service training.
