Job overview

We are looking for an experienced and motivated clinical pharmacist to work in our AAU Pharmacy team. The AAU Pharmacy team play a vital role in medicines optimisation as key members of the multidisciplinary team on the unit.  

You should be an innovative and enthusiastic team player who communicates effectively at all levels, and should hold a post-graduate clinical qualification and significant relevant clinical experience of working on a busy hospital ward and as part of a multidisciplinary team.

Main duties of the job

  • Provide excellent patient care through direct patient facing activities. 
  • Provide a 7 day pharmacy service as per agreed shift pattern (12 hour shifts)
  • Support transition of care of patients from AAU to other wards.
  • Provide leadership on matters pertaining to medicines optimisation, to advise on medicines use (risk, clinical governance, research and development, formulary, expenditure) and to develop/audit relevant protocols and guidelines.
  • Be responsible, on a day to day basis, for medicines reconciliation and medicines optimisation for patients on AAU
  • Provide a clinical service and support to SDEC (Same Day Emergency Care) unit
  • Undertake clinical supervision and training of junior staff
  • Provide support and in-reach to other ward areas where necessary

Education and qualifications

Essential criteria
  • Registered with General Pharmaceutical Council (GPhC)
  • Successful completion PgDip Clinical Pharmacy or equivalent qualification/experience
Desirable criteria
  • Independent Prescriber Qualification

Experience

Essential criteria
  • Minimum 3 years experience working on NHS ward
  • Experience working as part of an MDT demonstrating ability to influence prescribing
  • Service development work - leading on audits or project work
  • Demonstrates good evidence of medicines optimisation
Desirable criteria
  • Previous experience working on an admissions unit
  • Education and training of junior staff e.g. experience as a diploma tutor or Trainee Designated Supervisor
