An exciting opportunity has arisen within Ipswich East Community Team. We are looking for either an energetic Band 5 Occupational Therapist or Physiotherapist who is passionate, committed, innovative, enthusiastic and highly motivated to deliver high quality care within patient’s homes.
You will be committed to team work, delivering high quality, holistic patient care, have a desire for on-going professional development and ambition to mould and develop the services at a local level.
The therapy team are co-located within Constantine House Ipswich, and visits are scheduled geographically in the IP3, IP4 and IP5 post code area. You will be working with a caseload including neurological conditions, falls assessment and management, and palliative care.
The contract is a full time 37.5 hour permanent position.
You will be working alongside and supported by other Occupational Therapists, Physiotherapist, Community and District Nurses, Generic Support Workers.

To be an active member of a multi-disciplinary, integrated community team
To manage a defined caseload of complex individuals, with the support of the Band 6 occupational therapists, Physiotherapists and clinical Lead using evidence based/person centred principles to assess, plan, implement & evaluate therapy interventions.
To provide clinical supervision and supervise band 3 & 4 staff
We will provide support through in-house professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring teams in Health & Social Care and primary care.
Previous community experience is preferred; but training will be available to applicants who possess the personal qualities/ skills required.
You will need to be able to meet the needs of this service by being available to work flexibly between the hours of 08.00–20.00 on a rota basis over a 7 day period.

To manage a caseload of patients with a range of multiple and complex health needs.
To utilise clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
To formulate an individual management/treatment plan for each patient using wide range of advanced assessment, clinical reasoning and treatment skills, including health coaching and promotion of self-management skills.
To ensure interventions are evaluated and plans modified to ensure effectiveness and timely discharge for the whole therapy team.
To provide training and advice to patients and carers to facilitate their care plan in complex situations.
To adhere to agreed national, professional and Trust standards
