Job overview

An opportunity has arisen for a full time Therapy  Assistant Practitioner to join the Stowmarket Community Integrated Team.

The team are motivated and forward thinking consisting of Therapists, Nurses, Social Workers and Support Staff who work closely with other agencies.

The primary function of the team is to provide community based care both within the persons home environment and at base.

We aim to support timely discharge and admission avoidance through exercise programmes and equipment provision to improve function and promote independence as much as possible. Holistic assessment and informed choice being paramount.

Main duties of the job

Working within an integrated team to complete assessments of need, promote independence, plan interventions, provide equipment, support training needs.

Working closely with other agencies to support people within the community setting

Work with other members of the team to provide an urgent response to people in need for support due to change in condition, loss of function or have fallen.

Person specification

Knowledge

Essential criteria
  • • Ability to work on own initiative without direct supervision
  • • Knowledge of specific treatment protocols for a wide range of health and social care conditions
  • • Able to maintain clinical records and complete basic reports
  • • Ability to provide and receive complex and sensitive information
  • • Ability to exchange condition related information with multidisciplinary team, patients and carers
  • • Ability to manage communication difficulties/barriers
Desirable criteria
  • • Experience of group work with patients

Qualifications

Essential criteria
  • • Can demonstrate intermediate level of theoretical knowledge and clinical reasoning skills acquired through coursework, in-house training and experience to NVQ/Diploma Level 3 or equivalent
Desirable criteria
  • • Membership or special interest in a community care and integration
  • Assistant member of Professional group such as CSP or BAOT or working towards this

Experience

Essential criteria
  • • Experience as a Therapy assistant or experience at a similar level in a related field of health care / health and fitness sector
  • • Demonstrable experience of delivering tailored personal care with an associated level of senior responsibility
  • • Knowledge of medical conditions and functional needs within community setting or similar context
  • • Completion of Integrated Therapies assistant core and/or specific competencies
Desirable criteria
  • • Knowledge of community resources and whole systems/ pathways approach to care
  • • Experience of working across and liaising with variety of teams.
  • • IT literate
