Job overview

An exciting new planner role to join our developing and innovative Home First and Reablement service. Are you enthusiastic and motivated who thrives in a fast-paced and proactive team working environment? If so we look forward to hearing from you.

This is an exciting new role to support people to live their best life possible. Working directly alongside the Co-ordinators managing the case load of the service in the Locality. You will ensure that all visit plans are built on to the system and scheduled. Proactively update and maintain schedules of visits to patients to optimise the number of active cases.

You will provide an efficient, comprehensive clerical service for the Home First & Reablement team to ensure safe and efficient service delivery across the service area. You will ensure capacity is flexibly and efficiently used to ensure workloads are fully operational to meet the needs of the people being supported by the Home First and Reablement service, promoting a maximum level of service level function at all times. 

You will support the wider Integrated Community Teams (ICTs) to help meet the needs of patients, members of the MDT and members of the public.

The Reablement and Home First Team provide vital short-term services to people by helping them to regain or attain as much independence as they can in their own homes. 

Main duties of the job

Responsible for the timely and accurate uploading and inputting new cases on to the clinical system. Working directly with the Locality Lead and co-ordinators in planning resource in terms of staff capacity to meet the demands of the service. This will require a flexible and proactive approach to ensure delivery meets the challenging and changing demand. 

To provide a comprehensive and effective service which supports the safe delivery of the Home First and Reablement service. 

Acting as a key member of the team, the post holder works on their own initiative to maximise service capacity. 

As lead be responsible for proactively managing demand of the service to effect flow and service delivery. 

Liaising proactively with the Co-ordinators, Lead and Therapists to realise capacity and respond in the fast-paced service.

Provide complex and sensitive advice to the service lead regarding flow, demand and capacity

Person specification

Qualifications

Essential criteria
  • Educated to GCSE level in Mathematics and English to Grade C and above.
  • NVQ level 3 in administration or an appropriate equivalent qualification or experience.
  • RSA Typing Level 2 or equivalent qualification or experience.

LENGTH AND / OR NATURE OF EXPERIENCE

Essential criteria
  • Experience of unsupervised administrative work in a busy office environment
  • Experience of working in a customer-facing environment with evidence of telephone communication skills
  • Experience supporting teams or managers and setting up/running effective office systems
Desirable criteria
  • Previous experience of working with the NHS/local authority

PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE

Essential criteria
  • Skilled in Microsoft Office Packages, e.g. Word, Excel, and PowerPoint and use of computers and keyboards
  • Confident to liaise with other organisations and people as appropriate.
Desirable criteria
  • Previous experience and competency of an electronic patient records (e.g. SystmOne)
