To conduct a full nursing and holistic assessment specific to 
Coronary Heart Disease (CHD) assimilating complex information requiring analysis and interpretation in order to take appropriate action to help avoid worsening symptoms and subsequent hospital admission. This includes assessment of social circumstances, on-going patient management and health education, which includes medication and lifestyle advice.

Main duties of the job

We are looking for a highly motivated, enthusiastic nurse with proven specialist knowledge and experience to join our growing team! 

This is an exciting opportunity to improve the clinical outcomes and quality of life for our service users who have recently received treatment for their coronary artery disease (CAD).  Our role involves being able to act as a clinical expert for our primary and secondary health care teams and effective liaison with other community health care professionals.  We are seeking those with a high level of knowledge of cardiovascular disease (CVD) in order to provide clinical expertise in providing all aspects of coronary heart disease management, including audit and development of the service.

Qualifications

Essential criteria
  • Degree/diploma in Nursing
  • Registered Nurse with the NMC
Desirable criteria
  • Evidence of formal post registration learning in cardiac field.
  • The post holder will have sufficient knowledge acquired through (informal and formal) specialist training in order to be able to manage a caseload of clients with complex needs.

Experience

Essential criteria
  • Experience of working with people with long term conditions.
  • Experience in supervising / mentoring others.
  • Experience in presenting/teaching in a group setting.
Desirable criteria
  • Clinical educator training completed.

Other requirements

Essential criteria
  • Ability to work flexibly between the hours of 8.30am and 7pm Monday to Friday to deliver future service.
  • Current valid driving licence & access to car.
  • Able to manage the physical demands of the role including setting up of a practical and / or education session independently.
Desirable criteria
  • Knowledge of national exercise guidelines for patients with Coronary Heart Disease.
