Job overview

This is a great time to join the Community Assessment Team and there are various roles available to suit a range of professionals.

Our mission statement; ‘We are working collaboratively to help get you home or identify alternatives to an acute admission for the best possible outcome’.

The Community Assessment Team (CAT) is based at the Emergency Departments across Gloucester Royal Hospital and Cheltenham General Hospital.  The teams purpose is to provide a rapid assessment of patients to prevent admission to the acute trust. We work between the hours of 8am-6pm with some travel to our key services who we work closely with across Gloucestershire.

The post holder will be responsible for assessing, planning, implementing and evaluating high-quality, holistic patient care, within the Emergency Department under the supervision of a Registered Practitioner.

For new starters, there will be a 6-8 week supernumerary induction period and you will be allocated a mentor to support. 

Main duties of the job

  • To assess a cohort of patients within the Emergency Department (ED) from presentation through to discharge with support from a Registered Practitioner.
  • Through training and development educate GHNHSFT and colleagues about available discharge pathways and community services.
  • Working with colleagues, to risk assesses suitability for discharge and ensure that arrangements are appropriate and timely.
  • To communicate information upon discharge to multidisciplinary teams, external agencies, patients and carers.
  • To work with community services, ie CATU, GHC Rapid Response Team (RR).
