As part of community mental health transformation we are seeking to develop the community rehabilitation pathway across Gloucestershire Health and Care NHS Foundation Trust with a particular focus on individuals with treatment resistant psychosis and frequent admissions or long stays in inpatient units locally, and out of county, as well as individuals with 24 h. 

As part of community mental health transformation we are seeking to develop the community rehabilitation pathway across Gloucestershire Health and Care NHS Foundation Trust with a particular focus on individuals with treatment resistant psychosis and frequent admissions or long stays in inpatient units locally, and out of county, as well as individuals with 24 hour support whose placements are at risk of breaking down.

Prif ddyletswyddau'r swydd

As the Mental Health Navigator the main duties will include:

  • To act as mental health liaison for statutory and VCSE services
  • To facilitate access to GHC services and activities to meet the needs of service users and carers within statutory and non-statutory community and in-patient settings and services.
  • To improve pathways for people in mental distress who may be at risk of relapse or disengagement, by providing liaison between the Voluntary and Community Sector Services who work with, and engage with these individuals, and Statutory Mental Health Services
  • To participate in induction and support of new staff and students and to co-supervise junior staff.
  • To work alongside a number of service users on a one to one or group basis towards identified recovery goals 
  • Actively engage with service users including those that are placed out of area or in in-patient settings experiencing significant mental illness and complex psychosis

We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module “Preceptorship for Healthcare” (AP7026).

Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.

QUALIFICATIONS

  • NVQ level 3 or equivalent level of competency-based training in a relevant health or social care area PLUS theoretical and practical knowledge to diploma level equivalent, obtained through formal competency training, short courses and experience OR successful completion of approved associate practitioner training or relevant foundation degree or commitment to obtain within 2 years.

LENGTH AND / OR NATURE OF EXPERIENCE

  • Knowledge of relevant legislation e.g. Mental Health Act, Child Protection, Homelessness Reduction Act 2017
  • Experience in mental health or social care services at Bands 2, 3 or 4, ideally in a community setting.
  • Knowledge of the CPA process
  • Knowledge and understanding of care planning and risk assessment (completed an Approved course in relation to Risk Assessment).
  • Knowledge and understanding of using a Recovery based approach to care.
  • Knowledge and understanding of social inclusion approach
  • Clinical skills training (awareness of physical health and wellbeing).
  • Awareness of issues of diversity
  • Ability to work as part of a team
