This is a great time to join the Community Assessment Team and there are various roles available to suit a range of professionals.

The Community Assessment Team (CAT) is based at the Emergency Departments across Gloucester Royal Hospital and Cheltenham General Hospital.  The teams purpose is to provide a rapid assessment of patients to prevent admission to the acute trust. We work between the hours of 8am-6pm with some travel to our key services who we work closely with across Gloucestershire.

The post holder will be responsible for assessing, planning, implementing and evaluating high-quality, holistic patient care, within the Emergency Department.

For new starters, there will be a 6-8 week supernumerary induction period and you will be allocated a mentor to support. 

Main duties of the job

·       To carry out a specialist and advanced clinical assessment of patients within the Emergency Department (ED) from presentation through to discharge.

·       Develop treatment plans for acutely unwell patients that can be managed on a community pathway.

·       Working collaboratively with the other Clinical Lead, to lead and develop new clinical pathways in order to meet the constantly evolving ED demands, promoting the role of CAT.

·       Work alongside the other CAT Clinical Leads to support with the day to day team management, providing leadership, education and support to other team members and direct line management to allocated members of staff.

·       Provide clinical and professional leadership to other team members.

Detailed job description and main responsibilities

  • To work as a clinician within their professional scope of practice in responding to the assessments of patients presenting with mostly acute medical or MSK injuries.
  • Provide comprehensive initial clinical examination and assessments of patients in order to determine and deliver initial treatment and/or refer on to other appropriate Dr/clinician.
  • Support other Clinical Leads in recruitment, retention and performance related issues, including being regularly responsible for the supervision and mentorship for clinical support staff and students.
  • Operational management of service on a daily basis.
  • Through training and development educate GHNHSFT and colleagues about available discharge pathways and community services.
  • Working with colleagues to risk assess suitability for discharge and ensure that arrangements are appropriate and timely.
  • To communicate information upon discharge to multidisciplinary teams, external agencies, patients and carers.
  • To work with community services i.e. CATU, GHC Rapid Reponse Team (RR).
