An exciting opportunity has arisen within Gloucestershire. We are seeking to recruit Registered Nurses and Midwives from all backgrounds with a passion for working with children in the community.

Main duties of the job
· Responsible and accountable for assessing and implementing programmes of care from the Healthy Child Programme, in collaboration with the Specialist Community Public Health Nurse. This may also involve other evidenced based programmes of care in partnership with clients or other agencies to identify and address health needs.
· Progress the level of intervention as may be required according to identified risk factors which potentially could impact on a child’s health outcomes.
· Provide leadership and act as a role model by representing the values and beliefs of the nursing profession in addition to aligning to the GHC Values Framework.
· Assess global health needs of communities, and children to reduce disadvantages through ill health, developmental delay or social circumstances.
· Promote and monitor the health of all children, identifying physical, emotional/mental health needs, disability and problems with development or behaviour.

Detailed job description and main responsibilities
The Health Visiting Service is part of the 0-19 Public Health Nursing Service, provides health information, advice, support and interventions based on identified needs of a child, their family and their communities. This is often done in partnership with other professionals and agencies who work with or interact with children to ensure the best outcomes are achieved for the child. Promotion of health, prevention of ill health and protection of children are key priorities for the School Nursing Service.
The specific job purpose is to support the Public Health Nursing Team 0-19 years. In the delivery of services to ensure the health needs of the population are met, through developing and maintaining close partnership working with multi-agency teams such Early Years, SEND and Social Care.
The role of the Public Health Nursing Service is to deliver government programmes to improve the lives of children and their families. The Healthy Child Programme (DH 2009) is a cross-government strategy that is currently used to deliver a core universal programme, which is led by Specialist Community Public Health Nurses (SCPHN).
The Public Health Nurse is a Practitioner working with children and families within the community, working in an effective, safe and inter-professional manner with all agencies. The role includes supporting families and children with special educational needs and disabilities (SEND) and support their progression on the Gloucestershire Health and Care NHS Foundation Trust (GHC) policies such as the SEND Graduated Pathway for Gloucestershire, SCAAS pathway, Safeguarding, promotion of health, prevention of ill health & protection of children are key priorities. This is undertaken in partnership with other agencies.
The Public Health Nurse works under the instruction and supervision of a SCPHN – Health Visitor in accordance with GHC protocols and competencies.
The post holder will work in collaboration with Public Health Nursing colleagues and other partners and agencies who work with children, young people and their families to improve health outcomes by
· identifying health needs
· creating a personalised care and support package in partnership with the child
· analysing and evaluating outcomes
· documenting and reporting activity
The post holder will be responsible for an allocated caseload which will include representing and advocating the health needs of the child and providing information and challenge within a multi-agency environment.
Working in a designated community, the post holder will understand the health needs of a community, build capacity within the community and work with a secondary school community to improve health outcomes.
Working as part of a public health team, the post holder will support junior colleagues, offering professional judgement, advice and aid learning and development.
The role is community based and includes managing a caseload of children, caseload management is regularly reviewed and discussed with the SCPHN.
The post holder will work in collaboration with Public Health Nursing colleagues and other partners and agencies who work with children, young people and their families to improve health outcomes by
