Job overview

We are seeking a highly experienced registered nurse, who is capable of working autonomously within a multidisciplinary palliative care team, to promote and maintain the quality of life for patients with complex symptoms and needs, living with life limiting conditions within a community setting.  The successful candidate will also provide operational and clinical leadership to the community specialist palliative care nursing team. 

The proposed interview date is: 18th March 

 

Main duties of the job

To identify, organise and evaluate specialist intervention in response to complex needs on a daily basis. This includes identifying oncological /medical/palliative care emergencies, analysing clinical options and liaising with appropriate member of the multidisciplinary team. Supporting other team members in clinical decision making, as well as demonstrating professional and leadership skills in managing a busy service.

Detailed job description and main responsibilities

To contribute to the allocation and proactive review of all referrals within the service. To support and direct timely intervention of patient review and assessment. Allocate referrals to an appropriate team member. To work autonomously assessing patients and carers within the community with complex physical, emotional, social and psychological symptoms on a daily basis to enable an appropriate plan of care to be developed.

Qualifications

Essential criteria
  • Degree educated
  • Qualification in palliative care
Desirable criteria
  • Post registration study at Masters Level
  • Attended Advanced Communications Skills Training

Knowledge, Skills & Experience

Essential criteria
  • Registered Nurse with the Nursing and Midwifery Council. Significant experience in the relevant speciality.
  • Significant experience at Band 6 in specialist palliative care or caring for palliative care patients in a community or hospital setting.
  • Experience of communicating regarding complex, sensitive issues with patients, carers and professionals
  • Experience of managing and prioritising a variety of responsibilities
Desirable criteria
  • Significant knowledge of symptom management & associated management strategies

  • Ability to apply effective leadership skills, responsive to a variety of circumstances
  • Excellent interpersonal skills
Desirable criteria
  • Able to advocate for others, representing their views in an objective manner
