The Dementia Team have a part-time new exciting opportunity for a Dementia Associate Practitioner position within our ever expanding team to help support our patients living with Dementia.`
We are seeking a highly motivated and enthusiastic individual to work alongside the Dementia Team within Hampshire Hospital Foundation Trust, both in Winchester and Basingstoke Hospital.

Main duties of the job
The individual must be have previous demonstrable experience of Dementia Care within a care setting. They must have excellent communication skills and be dynamic in their approach in delivering person centred care to patients living with Dementia and supporting their family and carers.
