Job overview

This role is only open to applicants that currently work within an NHS Trust within the Hampshire and Isle of Wight Integrated Care System (HIOW ICS).

This post has been developed to support the Lead Cancer Nurse with the implementation of the personalised care agenda. This includes the implementation of the cancer recovery plan which encompasses Holistic Needs Assessment, implementation of End-Of-Treatment summaries, responsibility for the Supportive Care and Wellbeing Service, Patient Initiated Follow-Up and ensure processes are embedded to ensure on-going sustainability across the Trust.

To provide highly visible, strong, and accessible leadership, acting as a clinical resource and ensuring the sustained delivery of high standards of nursing care and service delivery, to nursing staff, patients and the public.

To work closely with the Macmillan Lead Cancer Nurse, Cancer Matrons and Operational Services Managers to support Cancer CNS’s in the delivery of key elements of the cancer strategy, providing assurance of quality standards.

To manage the wellbeing and supportive care service and ensure that local personalised KPIs are delivered, including PIFU, HNAs and end of treatment summaries.

 

Main duties of the job

 To lead and project manage the Living with and Beyond Cancer Programme of work across the Trust. This is to enable all cancer services to develop and embed an effective process for Personalised Care. This includes the HNA and care planning, Treatment Summaries, Health and Wellbeing events and Stratified Follow Up Pathways.


To lead and project manage eHNA via either Macmillan portal or EPR, across all tumour types to ensure implementation and evaluation, and move to business as usual across the organisation 


To lead and project manage PSFU via MyMR or appropriate alternative systems, across all tumour types to ensure implementation and evaluation, and move to business as usual across the organisation 


Support all MDTs to understand and implement End Treatment Summaries. 

To lead and manage the development of the wellbeing and supportive care service, ensuring that there is robust method of evaluating the service and provision of required reports that to can be shared with The Ark charity, in order to continue to secure funding.

To ensure the effective leadership and development of Lead Cancer Nurse Specialists including ensuring that all staff members have an annual appraisal and development review. 

To support the cancer CNS teams, supporting and developing them in their role to ensure they are able to provide effective day-to-day management and supervision of their staff.
