We are excited to announce new opportunities within our Hampshire and Isle of Wight All Age Continuing Care Learning Disabilities Team. Prif ddyletswyddau'r swydd

As a Senior Clinical Assessor/Practitioner) in Learning Disabilities, you will be responsible for conducting comprehensive individual assessments to assess eligibility for NHS Continuing Healthcare (CHC). We are seeking professionals who are proficient in case management and reviewing of care and support for people with Learning Disabilities that are CHC eligible or have S117 aftercare entitlement under the Mental Health Act 1983. You will enable timely provision of appropriate care and support ensuring that people’s needs are effectively met whilst promoting Personal Health Budgets to encourage independence.

You will need to have excellent interpersonal, presentation and communication skills and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and supported living environments.

The All Age Continuing Care team is friendly and supportive spanning a wide range of people who work together to support each other, patients and the wider system. We support over 6,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, children’s continuing care and other joint funding and oversight of a budget of £247M. Following the re-design of our services, we have Senior Clinical Assessor/Practitioner posts available in Hampshire. The role is community based with flexible working from a local office, The purpose of our All-Age Team is to Improve health opportunities and outcomes within our system by delivering seamless, affordable, and individually focused care for those requiring our services. Whilst experience in Continuing Healthcare would be helpful, if you have experience of working with clients with learning disabilities and the right skills and attitude, we can support you to develop the specific skills required for Continuing Healthcare. We offer in house training opportunities and Team Leaders who are non-case holding in order to mentor and develop our Clinical Assessors with complex cases, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team, comprising administration, brokerage, commissioners, Deprivation of Liberty experts and case practitioners who provide a wide range of expertise and support.

You will be highly organised, flexible and have a positive ‘can do’ attitude.

Be highly motivated and keen to learn/develop (training in respect of the NHS CHC Framework and its application, Safeguarding, MCA, Court of Protection and Personal Health Budgets will be provided as part of induction / ongoing development).

Be accustomed to a high degree of autonomy on a day-to-day basis.

Be able to assimilate large amounts of information and have excellent verbal and written communication skills, with attention to detail.

Be able to provide accurate, detailed and comprehensive reports and ensure patient files are kept up to date as per professional standards.

Ability to develop relationships and foster effective networks with key stakeholders, families and carers and have developed people management skills with experience of resolving difficult situations and challenging individuals effectively and calmly.

Be comfortable working with a wide variety of ages and individuals with very different clinical presentations

Be able to assess and prescribe care dependent on the individuals holistic health and care needs

Have a strong value set around quality and patient/carer experience

Be a key member of the team and participate in team meetings, supervision and appraisals.

Have excellent time management skills and thrive on working at pace.

Have excellent knowledge of Microsoft office and have intermediate typing skills.

Be able to travel daily across the county and countrywide. Please note a valid driving license is essential for this post.

Knowledge, Training and Experience

  • Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE. including AMHP’s and mental health social workers or significant relevant experience (LD/MH/LDAP teams only).
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
  • Experience of assessment of health and social care needs – care planning and coordination.
  • Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles).
  • Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team).
  • Ability to work effectively under stressful conditions in a rapidly changing environment.
  • Good knowledge of Mental Capacity Act and relevant legislation (for DOLS team)
  • Experience in communications and stakeholder management.
  • Effective working knowledge of Microsoft software.
  • A good understanding of the health and social care environment and roles and responsibilities within it.

  • Knowledge of and experience of NHS Funded Continuing Health Care criteria – guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act).
  • Experience of developing care packages.
  • Experience of implementing and managing Personal Health Budgets.
