To work within the Specialist Community Learning Disability Team offering dietetic assessment and intervention for adult service users with a learning disability. This will be undertaken in partnership with:

• The individual
• The individual’s carers / family members
• Members of the multidisciplinary team

The post holder will be line managed within the Buckinghamshire and Hertfordshire  Learning Disability Team and will be professionally accountable to the Specialist Learning Disability Lead – AHPs.

Buckinghamshire and Hertfordshire Learning Disability Partnership team are looking for a highly specialist Dietitian with a passion for delivering the highest standard of care for individuals that have a learning disability. 

You will be working as a specialist Dietitian within our Community Learning Disability Team based at Buckinghamshire or Hertfordshire. 

Our Community Learning Disability Teams provide person centred and holistic assessment and care for adults with a learning disability living in the community. The multidisciplinary team works with individuals that live independently in the community, with family/carers, and those that reside in supported living schemes or 24-hour residential placements. The service is committed to improving the health and well-being of people with a learning disability across Buckinghamshire and Hertfordshire, by working in a simple, person-centred way that empowers individuals to maximise their potential.

The Role:

• Managing a caseload, using evidence-based person-centred principles to assess, plan, implement and evaluate interventions in clinic and community settings
• Providing dietetic assessment and treatment to service users who present with nutritional concerns
• Working in an interdisciplinary way to assess service user’s needs, devising care plans, and evaluating outcomes.
• Provide support and education to service users, carers, and members of the multidisciplinary team regarding nutrition and hydration.
