Job overview
An exciting opportunity has arisen within Mid Essex to be part of a Physical Health Team providing services to people with Severe Mental Illness. HPFT are looking for a Registered Nurse with a passion for improving the physical health and wellbeing of people with mental health difficulties, within the local community.
Main duties of the job
The post holder will lead a small team of staff working in GP practices undertaking annual physical health checks for people with a Severe Mental Illness. You will have lead responsibility for providing a comprehensive and systematic annual health check to patients on the SMI registers, provide nursing leadership and clinical expertise for the physical health team, developing working partnerships with primary care and ensuring best practice within mental health and physical health promotion.
You will provide clinical leadership to the team and be the key point of liaison with the GP practice staff and local services.

Qualifications & Experience
Essential criteria
- Registered nurse
- At least 3 years experience at Band 6
- Community nursing experience
- Venepuncture competent.
- Ability to identify and intervene for health and lifestyle factors
- Evidence of continuing professional development.
Desirable criteria
- Experience of working with people with severe mental illness
- Experience of working in other clinical settings.
- Smoking cessation training

Essential criteria
- Car Driver (unless you have a disability as defined by the Disability Discrimination Act 1995)
- Ability to travel and work flexibly across different sites in Hertfordshire
- IT Skills; including the use of Microsoft Office and Outlook, Microsoft Teams, entering data onto electronic patient records
Skills and aptitude
Essential criteria
- Ability to communicate effectively within a multi-professional team.
- Confidence to work autonomously, use own initiative, and make decisions in complex situations.
- Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassuring skills are required.
- Ability to deal with distressing circumstances and challenging behaviour.
- Experience of risk assessment and management.
- Ability to cope with an unpredictable work pattern and frequent interruptions
- Understanding of legal framework MHA, MCA, DoL, safeguarding and social factors involved in care.
- Facilitate Clinical Supervision of band 4 (e.g Health Care Assistants)
Desirable criteria
- Specific clinical skills applicable e.g. management of tachycardia for patients on Clozapine
- Implementing quality improvement initiatives.
