Job overview

We are looking for a dynamic and patient focused individual to join our Community Therapy Team.

You will work alongside the Locality Manager and Deputy Locality Manger to provide leadership, managing your team, ensuring patient flow, and identifying and taking forward quality improvement.

You will work as part of the Integrated Community Team (ICT) pathways to prevent inappropriate hospital admissions and to provide advanced holistic assessment/diagnostics, support, treatment and rehabilitation of patients in their own home

You will work closely with the Lead Nurse, Clinical Lead and other members of the Senior Team to continue to develop the service. You will be required to travel to various locations within Hertfordshire Community NHS Trust to attend meetings that cannot be held virtually. 

 

Main duties of the job

You will provide clinical leadership within the unit, ensuring all members of the therapy team deliver high quality clinical care within scope of professional practice and work within the competency framework.

You will support the therapists with managing their caseload, goals setting and meeting the estimated discharge dates to maintain patient flow through the unit.

You will support and continually develop your therapy team, encouraging training, managing sickness, addressing performance issues, and undertake recruitment of new staff.

Through audits and feedback you will work with the  locality manager and Deputy Locality Manager , clinical quality lead and senior operational team to take forward continuous improvement.

Detailed job description and main responsibilities

You will support your team with day to day management of their patients.

You will work alongside the rest of the MDT to ensure patients achieve their goals.

In managing the therapy team you will monitor performance: addressing issues as they arise, complete appraisals, manage sickness, support compliance with mandatory training.

You will work with the Locality Manager and Deputy Locality Manager to monitor performance of the unit and continue to implement identified changes to meet CQC requirements

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £1,915 per annum pro rata.

You must hold a full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Person specification

Qualifications and Training

Essential criteria
  • Diploma/Degree in Occupational Therapy or Physiotherapy
  • Registered with Health Professions Council (HPC)
  • Evidence of Post graduate courses relevant to specialty
  • Evidence of continuing professional development

Experience and Knowledge

Essential criteria
  • Appropriate Post Graduate experience with the ability to demonstrate skills appropriate to the role
  • Experience of working in a related field
  • Experience or working in the NHS
  • Experience of working in close collaboration with health and social services colleagues.
  • Presentation and teaching to staff team and other groups

Skills and abilities

Essential criteria
  • Occupational Therapy or Physiotherapy assessments and interventions
  • The ability to analyse assessment results to formulate treatment plans, taking into consideration client, social and environmental factors
  • Excellent multi-disciplinary/agency working skills
  • Computer skills as required for the effective execution of duties and responsibilities
  • Good written and spoken English
