An exciting opportunity has arisen for a highly motivated and innovative Speech & Language Therapist (SLT) to join the Norfolk Forensic Community Learning Disability (NFC-LD) team based at Little Plumstead Hospital.

The successful candidate will work as an influential and integral member of a diverse multi-disciplinary Learning Disability and Forensic team; working across a range of community settings throughout Norfolk with adults with speech, language and communication difficulties. A key feature of the team is its close working relationship with the on-site medium secure inpatient learning disability service, the Broadland Clinic. The post holder will have the unique and exciting opportunity to work across both inpatient and community services.

You will be forward thinking and able to demonstrate clinical knowledge in the management of communication difficulties, based on current evidence.  You will autonomously undertake specialist assessments & interventions and be actively involved in the continued development of the NFC-LD communication pathway. The person successful in their application will be an essential member of the MDT team, working alongside Medical, Nursing, Occupational Therapy, Psychology, Arts Therapies, and Social Care colleagues to support our service users achieve their goals.

·       To manage a caseload using evidence-based, person-centred principles to assess, differentially diagnose and advocate for people with communication difficulties, and plan, implement and evaluate interventions in community settings

 

·       To undertake supervision of junior staff and students as required

 

·       To work as a member of the multidisciplinary team

Duties and Responsibilities:

 

·       To manage a  caseload, using evidence based person centred principles to assess, plan, implement and evaluate interventions for the specified service user group

·       To work as a core component of the multidisciplinary team in line with specialist pathways and policies

·       To maintain clinical records

·       To develop skills and knowledge 

Clinical Responsibility

 

·       To assess, diagnose and develop treatment plans for service users, including those with: learning disabilities, additional sensory and/or physical disabilities, neurodevelopmental conditions, mental health diagnoses and challenging behaviour

 

  • To be responsible for managing a defined caseload , in the context of the service caseload,with ongoing formal evaluation of clinical effectiveness using specified outcome measures

 

  • To work with and be available to other SLTs and Assistant Therapy Practitioners (ATPs) for support and joint consultation regarding clinical practice as required

 

  • To be an autonomous practitioner

 

  • To work with service users and carers to identify SLT requirements as part of the overall care plan

 

  • To plan and implement individual interventions, in collaboration with the service user and carer

 

  • To monitor, evaluate and modify interventions with service users to ensure effectiveness of intervention and positive outcomes

 

  • To have knowledge and understanding of the application of alternative and augmentative communication

 

  • To have the skills needed to facilitate group work with service users

 

·       To undertake complex risk assessment and risk management for individual clients

 

·       To ensure that clients are consulted and involved in decision about their care

 

Leadership and Staff Management Responsibility

 

·       To review and reflect on you own practice and performance through effective use of professional and operational supervision and appraisal

 

·       To exercise good personal time management, punctuality and consistent reliable attendance

 

·       To demonstrate emerging leadership skills through the management of designated projects

 

·       To promote awareness of the role of SLT within clinical services, negotiating priorities where appropriate  

 

·       To participate in the induction, training and education of students and other staff as appropriate

 

·       To apply knowledge, experience and skills to make clinical and safeguarding decisions in the absence of senior staff

 

Financial Responsibility

 

·       To be responsible for maintaining stock, advising on resources to carry out the job

 

·       To be responsible for the safekeeping and use of equipment and materials

 

·       To ensure Health and Safety Regulations and Data Protection Act Regulations are observed and instituted

 

Service Development and Improvement

 

·       To participate in the planning, evaluation and audit of practice, clinical pathways and protocols within your area

 

·       To contribute to the Trust’s Practice Governance arrangements

 

·       To challenge practice, in the best interest of service users, carers and the Service

 

Communications

 

·       To establish and maintain robust networks of communication with HPFT and partner agencies

·       To monitor and evaluate treatment in order to measure progress and ensure effectiveness of the intervention

 

·       To work as a member of the multidisciplinary team, contributing to decisions with regard to client care programmes

 

·       To provide support and education to clients, carers and members of the multidisciplinary team regarding aspects of SLT

 

Analytical and Judgemental Skills

 

·       To demonstrate commitment to ongoing personal development and acquisition of further skills and knowledge elating to clinical areas as specified and the application of these skills to clinical practice, and through participation in internal and external development opportunities, recording learning outcomes in a portfolio

 

·       To comply with the RCSLT Code of Ethics and Professional Conduct and national and Trust procedures

 

·       To respect the individuality, values, cultural and religious diversity of clients and contribute to the provision of a service sensitive to these needs

 

·       To review and reflect on own and others practice and performance through effective use of professional and operational supervision and appraisal

 

·       To adhere to local and national standards and guidelines relating to professional practice and maintain registration with the Health and Care Professions Council

 

Planning and Organisational Skills

 

·       To plan own diary and appointments with the service users and carers

 

·       To assist in the identification of training needs of junior staff including that needed in response to new trends in practice and evidence base

·       To ensure that up to date written and electronic records and reports are maintained in accordance with Professional and Trust standards

Physical Working Conditions and Environment

  • To work within community settings; including the homes of service users, and the Broadland Clinic (medium secure inpatient LD & Forensic service situated on the Little Plumstead Hospital site)

 

Additional Information

 

It is inevitable that the post holder will be required to deal with distressed people and find themselves in challenging and / or risky situations as they will be working with people with learning disability and mental health problems / challenging behaviours.

 

 

Professional Registration

 

It is a condition of employment that you maintain your registration with appropriate body. You are required to be aware of and comply with the policy and requirements of that professional body. It is your responsibility to ensure that your registration is renewed when necessary and that you formally confirm all renewals with your manager.

 

Knowledge and Skills Framework

 

The post holder will be expected to meet the requirements of the NHS Knowledge and Skills Framework (KSF) appropriate outline for the post.
