Are you a registered mental health nurse, occupational therapist or social worker?  We have a unique role for a Senior Discharge Care Co-cordinator.

Are you interested in working with an outstanding NHS Trust to deliver community treatment in mental health?  

The Senior Discharge Care Co-ordinator provides the interface between acute inpatient services and community services in Adult Acute and Urgent Care Services.

The role is part of the friendly Enhanced Discharge Team / Integrated Patient Flow Team and be supervised by a bed manager and clinician. They will become part of the Integrated Patient Flow Team for day to day support. 

Kingfisher Court and the other inpatient services promote agile working and the Senior Discharge Care Co-Cordinator can base themselves in the various ‘hot desk’ facilities.

The Senior Discharge Care Co-coordinator, through the established links with the Integrated Patient Flow Team will be able to liaise with Discharge Co-coordinators and the Housing Worker in order to progress actions for service users from the community teams in Adult Acute and Urgent Care Services.

You must hold a full valid driving licence and access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

The successful candidate will:

  • be a registered mental health nurse with NMC, or registered social worker with Social Work England
  • have relevant experience with mental health services
  • attend inpatient bed flow meetings and work closely with the Discharge Co-ordinators in identifying barriers to discharge
  • establish any potential delays to discharge
  • liaise to establish anticipated outcome from admission with Care Co-ordinator and duty community teams
  • be responsible for review of progress to discharge on a daily basis and co-ordinate clinically ready for discharge service users.

Qualifications

Essential criteria
  • An appropriate professional nursing / social work qualification
  • Education to degree level
  • Training in principles and application of outcomes focussed assessments and personalization
  • Relevant post-qualifying training for Nursing /Social workers
  • Registration with NMC /Social Work England
  • Training and experience in Safeguarding
Desirable criteria
  • Approved Mental Health Professional/Best Interests Assessment Training

Experience

Essential criteria
  • Extensive post qualification experience
  • Experience of undertaking assessments, focused as personalisation, choice and control, and care planning
  • Understanding and experience of personal budgets and direct payments
  • Experience of multidisciplinary working and ability to work collaboratively
  • Experience of partnership working with service users and carers, demonstrating dignity and respect
  • Knowledge of legislation, theory, and policy in relation to relevant care group
Desirable criteria
  • Experience of work as Approved Mental Health Professional and/or Best Interests Assessor

COMMUNICATION SKILLS

Essential criteria
  • Excellent ability to communication with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
  • Ability to manage highly sensitive information
  • Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports

ANALYTICAL SKILLS

Essential criteria
  • Ability to analyse service user and carer information.
  • Experience of undertaking small scale audits and surveys.
