·  The Pharmacy and Medicines Optimisation team within HPFT provides dispensary and clinical pharmacy services to mental health, learning disability and CAMHS services on the Kingsley Green site, to other units and community-based teams across Hertfordshire and beyond, where HPFT is contracted to provide a service.

 

·       The post holder will contribute to the work of the Pharmacy and Medicines Optimisation team to deliver the standards set by regulators and professional bodies such as the Care Quality Commission (CQC), Medicines and Healthcare Products Regulatory Agency (MHRA), General Pharmaceutical Council (GPhC), Royal Pharmaceutical Society (RPS) etc.

 

·  The service will be provided to all medical, nursing, managerial, pharmaceutical and clinical professionals working in Mental Health and Learning Disabilities in designated units for which the post holder will be responsible.

Overview of Main Responsibilities:

 

  • Support the pharmacy and medicines optimisation team with the delivery of a legal, safe, high quality pharmaceutical service.
  • Support the development of medicines optimisation standard operating procedures and pharmacy service protocols.
  • Timely completion of operational duties such as stock control, date checking of medicines and temperature recording.
  • To be fully conversant with all local pharmacy protocols
  • To assist with daily dispensing of controlled drugs and the update of Controlled Drug register.  
  • Assist in the medicine reconciliation process by supporting completing medication histories using the most recent, relevant sources available.
  • To organise and provide a supply of stock and non-stock pharmaceuticals and develop / manage medicines management systems to ensure the efficient, safe and secure handling of medicines.

Clinical/Technical

 

·       Ensure safe and appropriate use of medicines to maximise benefits and minimise risk to service users. This includes medicines reconciliation, regular review of prescriptions, identification of significant drug interactions and adverse effects, advice on clinical monitoring, compliance with legal aspects (e.g. Medicines Act, Misuse of Drugs Act, Mental Health Act) and compliance with Trust policies and guidelines.

·       Ensures security of stock drugs are maintained.

·       Accurately dispense prescriptions for inpatients and leave/discharge requests using the Pharmacy IT system – select, compile, package, label and record prescription items according to departmental procedure to facilitate and ensure the safe and effective administration and use of medication.

·       Carry out comprehensive medicines reconciliations on the wards by communicating effectively with service users, carers, ward staff, doctors, other Trusts and community services, including but not limited to GP surgeries, community pharmacies and community drugs teams to obtain up to date and accurate information regarding patients’ medication history.

·       To provide information on common drug dosage and use. Advise service users, carers, clinical staff on issues relating to medication regimes and changes to

prescribing.

·       To act as an Accredited Checking Pharmacy Technician (ACPT). Involves final checking clinically screened prescriptions and orders prepared by other pharmacy staff, before release to the service user, ward or unit.

  • Accurately assess the safety and suitability of patients own drugs (PODs) to ensure correct use in hospital in accordance with the relevant procedures.
  • Conduct a thorough technical medication review of service user’s medication chart, order and supply medicines at ward level using the correct documentation.  Report any missed, delayed, or omitted medicines in accordance with relevant procedures.
  • Check for prescribing and other errors within the limits compatible with the pharmacy technician skills and escalate concerns to the attention of the clinical pharmacist or prescriber in accordance with the relevant procedures. 
  • Maintain the controlled drugs register, making entries, checking, and reconciling stock as appropriate.
  • Assess suitability of pharmaceutical products for return of stock.  Return items using the Pharmacy IT system, dispose of according to departmental procedure.
  • To use pharmacy stock control and dispensing programme (Ascribe) and other computer systems in the departments throughout the trust and external units, receive stock, generate relevant paperwork and labels as appropriate.  Update electronic files involving confidential service user information.
  • Participate in stock management including ordering, receipt, storage, expiry date checks, distribution and returns.  Assist the lead procurement technician in resolving any stock discrepancies.
  • Identify, dispense and distribute stock pharmaceutical items including Controlled Drugs to wards and departments throughout the Trust according to Standard Operating Procedures (SOP’s)
  • To support the dispensary manager in coordinating and managing the day-to-day running of operational dispensary pharmacy services.
  • To maintain clean and tidy dispensing facilities and safe systems of work including expiry date checking and correct disposal of medication.
  • To maintain accurate data relating to stock balance and assisting investigations when

discrepancies occur.

  • To ensure that all medication incidents are recorded on DATIX by staff operating in the dispensary.
  • Take part in counselling and provide information on medicines to service users, healthcare staff and doctors.
  • Deal with telephone enquiries.
  • To provide mentoring and training support to new and junior members of staff.
  • Visit wards to carry out stock top-ups.
  • Take an active role in the pharmacy team and provide support to less experienced staff.
  • Seek advice from senior pharmacy colleagues as appropriate to provide advice to clinicians.
  • To undertake Continuing Professional Development in order to keep pharmaceutical knowledge current, ensure best practice and comply with requirements in line with GPhC professional standards.

 

Leadership and Staff Management Responsibility

 

·       To uphold and embody the HPFT trust values.

·       To supervise and train less experienced technicians, students, and other staff.

·       To assist in the training of pre-registration trainee pharmacy technicians (PTPTs), via the apprentice routes including new posts & present staff that may wish to uplift their skills under the supervision of the training lead for PTPT.

·       To be an Accredited checking technician (ACT) facilitator or be willing to undergo training to be an ACT facilitator for pharmacy technicians undergoing their ACT training.

·       To commit to personal self-development and training reflecting the needs of the post and the department as identified through performance appraisal.

  • Support the training / induction requirements for other staff as appropriate.
  • Plans the rota for the technical pharmacy team when required.
  • To line-manage relevant pharmacy staff including conducting annual PDRs (appraisals) and regular supervision sessions.
  • To follow all Trust policies relating to management including but not limited to sickness absence, grievances, capability.
  • To take an active role in helping to lead the dispensary team, providing support to junior and less experienced members of staff.
  • To be an integral member of the dispensary team and to work with other senior technicians to manage the staffing resources to ensure an efficient and comprehensive service is provided all times.
  • To deputise for other pharmacy staff as necessary
  • To plan and organise dispensary activities, re-allocating tasks as necessary.

·       Managers have a duty to promote and support staff’s personal health and wellbeing at all times, this includes both physical and mental health and wellbeing.

·       Managers have a responsibility to support staff by directing employees to services that are available to everyone to help in managing health and wellbeing.

·       They should be open and approachable as well as proactive in discussing and agreeing a process to monitor an employee’s mental and physical health.
