Job overview

We are recruiting for a Specialist Pharmacist Thrombosis who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

We are looking for a dynamic, motivated and forward thinking pharmacist to work across the Trust to lead the VTE Quality Improvement Project, work with the Thrombosis Multi-disciplinary team and the pharmacy team.

This is a full time post (37.5 hours per week) and includes working weekends and on-call on a rota basis in the pharmacy. We also welcome part time and job share applicants.

We will fully fund and support you to undertake relevant post graduate training including the prescribing qualification and leadership training. 

We will develop your portfolio of skills to prepare you for more senior roles within the department through involvement in directorate and project work focused on improving quality of clinical services and patient-focused care.

Main duties of the job

To operate as specialist pharmacist for thrombosis including leading the Trust wide quality improvement project into VTE.

To participate in service development and demonstrate use of critical evaluation skills. This includes the undertaking of root-cause analysis for patients who develop healthcare-associated thrombosis; critique the thromboprophylaxis provision they received; feedback results to the relevant Directorate teams and support the preparation & publication of a quarterly report on the data collected.

Develop, implement, monitor and evaluate protocols and guidelines for anticoagulant therapy. These may be used both Trust-wide and external to the Trust.

Organise, deliver, evaluate and feedback audit and project work with respect to anticoagulant use

Develop and maintain Patient Group Directions regarding anticoagulants where appropriate.

To support the delivery of the Trust Thrombosis Committee agenda

To be a clinical mentor and guide junior pharmacists, foundation trainee pharmacists throughout their training programmes.

Qualifications and Training

Essential criteria
  • Master Degree MPharm
  • Competency assessed and examined professional registration MRPharmS
  • Mandatory CPD to maintain fitness to practice
  • Pharmacy Practice or equivalent or successful completion of General Practice Diploma Core Course and examination or equivalent experience

Previous Experience

Essential criteria
  • Appropriate post qualification experience in hospital pharmacy
  • Previous experience in providing clinical services to wards
  • Medicines Information experience
  • Computer literacy – Word/Excel
  • Experience of working as part of a multi -disciplinary team
  • Proven experience of audit

Skills

Essential criteria
  • Excellent communication skills verbal and written
  • Able to work alone or in a team
Desirable criteria
  • Can show appropriate applications of clinical knowledge and effective pharmaceutical care skills

Knowledge

Essential criteria
  • Good Clinical Knowledge at consultant level
  • Up to date knowledge of national pharmacy issues
