Trainee Advanced Care Practitioner

We are looking to recruit a trainee Advanced Care Practitioner to join the  Hospital at Home team within the community.  This will be across the East & North corridor. 

The service has evolved to meet increased and changing patient need and clinical priorities. To support this we are developing a new workforce model to include Advance Care Practitioners (ACP). This is an exciting opportunity for experienced community practitioners to progress further in their clinical career either nursing or allied health professional. We are always seeking to collaborate and improve with others and would love you to join us on our journey.

The successful candidate would be expected to complete a Masters accredited university course to become a fully qualified ACP. We would also  welcome applications from practitioners who have completed a non-accredited pathway/course but who are willing to complete the e-portfolio route to become a fully accredited ACP. Individuals who have partially completed some relevant Masters levels modules will also be considered for a condensed/personalised pathway.

The hospital at home service facilitates the care of patients at home where their condition can be managed appropriately and safely within the home. This is a 7 day service from 8am - 8pm with the post based in the East or North Corridor.

This post is also open to allied health professionals as well as nurses.

Prif ddyletswyddau'r swydd

  • Treat patients within their own home
  • Work with the community teams to ensure high level of clinical care
  • Prescribe/review medication as per required/appropriate for patients in the caseload.
  • Interpret/request investigations.
  • Develop medical treatment plans for patients on the caseload.
  • Coach staff seeking advice on patients' suitability for the hospital at home pathway.
  • In reach into acute trusts as part of their training programme to teach them about hospital at home pathways and patients who would benefit from it.
  • Work alongside the project development team in HCT to further develop hospital at home in line with DOH guidelines and the latest research available.
  • This is an evolving role and the ACP will be expected to work collaboratively within the team to help steer future developments and support the education of other trainee ACPs.

The post holder will work towards gaining ACP accreditation and once able apply advanced clinical expertise, levels of judgement, discretion and decision making within the face to face element of the hospital at home service. As part of the training process they will work alongside more senior/experienced clinical professionals in the team and once able they will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with complex presentations and multi pathologies.

The role will provide an expert resource, providing a visible clinical profile, having both advisory and clinical input into patient care within the hospital at home service. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

Works collaboratively with the multi-disciplinary team including medical, nursing and allied health staff to manage medical conditions and to plan and implement treatment for illnesses.

Once an Advanced Clinical Practitioner, this role will provide education to other staff and students within the clinical area.

Qualifications and Training

  • Currently registered practitioner with relevant Professional Governing Body
  • First Degree or equivalent experience
  • Ability to study and complete Master’s Degree in Advanced Clinical Practice within the required timeframe. This includes both academic and clinical criteria to become an ACP
  • Teaching and Assessing Qualification
  • GCSE Level 4 or equivalent in Maths & English

Experience, Skills and Knowledge

  • Needs significant post registration experience commensurate with being an Advanced Clinical
  • Evidence of leading organisational and management change
  • Able to work a minimum of 30 hours per week (employment contract) in order to join MSc ACP apprenticeship programme and ensure ability to complete all elements
  • Able to work independently, make decisions, organise and prioritise own workload
  • Ability to network with other ACPs and clinical colleagues, both within and external to HCT
  • To have highly developed dexterity, precision and eyehand co-ordination skills
