We have a rare & exciting opportunity for an experienced & passionate Learning Disability nurse to join  the Intensive Support Team ( IST )in the Team  leader band 7 position within the West Specialist Learning Disability Service.

 

Main duties of the job

The Team Leader role in IST involves working with adults with a learning disability and challenging Behavior and/  or mental health issues in the community with the aim to prevent an admission to hospital

The Team leader role requires a dynamic and enthusiastic Learning Disability nurse who has experience   of working in the community and an understanding of the Transforming Care Agenda  and a passion for working with the individual, their carers and  networks  to support individuals in their  own home and  explore  alternatives to hospital admission

 

 

Detailed job description and main responsibilities

To achieve this you will be leading the Intensive support nurses and senior support workers & your role will include

  • Supervision of band 6 nurses  & monitoring of band 5 & band 4 supervisions 
  • Excellent communication and organizational skills
  • Operational management of  IST including annual leave, e-roster  
  • leadership of student nurse placements and recruitment
  • Professional  responsibilities  including CPD  & preceptorship.
  • Monitoring of Caseloads and case management to try and help prevent admission.
  • Screening and allocating referrals 
  • Charing Nurses meetings and prevention of hospital admissions 

 

Person specification

Qualifications

Essential criteria
  • RNLD /RNMHH
  • ENB 997/998( Mentoring & Preceptorship) or equivalent
  • Post graduate qualification

Experience

Essential criteria
  • At least 2 years’ experience of working in a Specialist LD community Service.
  • Clinical and operational management of junior staff
  • Managing service priorities met
