Job overview
The Discharge to Assess (D2A) Team has recently merged with our Bed Based teams across our 3 Community Hospitals. This is a hybrid role working as part of the D2A team but embedded within the inpatient team at Queen Victoria Memorial (QVM) Hospital. You would be joining a supportive team of qualified therapists and therapy support workers to provide quality care to our D2A patients within care homes in and around the East & North Hertfordshire area, as well as the inpatients at QVM.
You will be based at QVM in Welwyn, Hertfordshire. However, when providing the D2A service, the post holder will be required to travel between care homes within East & North Hertfordshire including, Stevenage, Hatfield & Broxbourne.
QVM is a 21 bedded unit located on the outskirts of Welwyn Village, Hertfordshire. QVM provides rehabilitation for patients with a wide range of conditions such as falls, frailty, stroke, orthopaedics and amputees. QVM is a purpose built unit to also support East & North Hertfordshire's plus size pathway. Rehabilitation is provided on a generic 17-day pathway, or a 35-day pathway for stroke patients.
Full-time and part-time hours will be considered.
Main duties of the job
· To assist qualified allied health professionals, including occupational therapists and physiotherapists in the delivery of rehabilitation in a variety of settings.
· To work without direct supervision, but under the direction of the qualified staff in the performance of specific tasks for an individual or group, as part of their treatment and rehabilitation programmes. This may be in a community setting such as the patient’s own home. It will include both individual one to one sessions and group sessions.
· To undertake routine assessment of people with acute conditions using investigative and analytical skill, under guidance of a qualified allied health professional.
· To formulate and deliver a standard individual management and or treatment programme, under guidance of a qualified therapist.
· To teach the use of walking aids, exercise equipment and aids and adaptations, including assessment and teaching of safety factors.
· To undertake routine wheelchair assessments and referrals, under the direction of a qualified member of staff.
· To carry out access visits into service user`s homes and collate and communicate relevant information and measurements back to the therapy team.
· To order any necessary equipment for service user’s as identified during the assessment period under the guidance, as required, of a qualified allied health professional.

Qualifications
Essential criteria
- Relevant Foudation Degree /HND in Rehabilitation or Relevant NVQ III
Desirable criteria
- Evidence of continuing professional development in relevant areas
Experience & Knowledge
Essential criteria
- Experience of working with healthcare professionals in providing rehabilitation
- Experience of working with elderly patients with a range of health conditions
- Knowledge of the rehabilitation process including common components of treatment packages, goal setting and use of outcome measures
Desirable criteria
- Experience of working in the NHS
Skills & Abilities
Essential criteria
- Therapeutic moving and handling skills
- Ability to accurately document therapy interventions in English
- Ability to liaise with other health and social care professionals and handover key information
- Ability to communicate sensitive information to service users and adjust communication style to individual needs

Essential criteria
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
